摘要
心力衰竭是心血管疾病的终末期阶段,预后差,且治疗手段有限。中医“心脾相关”理论认为心与脾在生理上相互依存、病理上相互影响,心力衰竭患者常伴随脾胃功能失调,形成心气亏虚和痰瘀内阻的恶性循环。现代研究揭示心力衰竭患者存在肠道菌群紊乱、代谢物异常及免疫炎症激活,从而加剧心肌损伤。健脾法作为中医治疗心力衰竭的核心策略,可通过益气、温阳、理气、活血等途径调节肠道菌群,改善代谢紊乱,抑制心肌纤维化,发挥多靶点协同作用。本文从“心脾相关”理论出发,系统阐述健脾法干预心力衰竭的中医病机逻辑与现代病理机制契合点,提出健脾法在不同心力衰竭证型中的个体化应用及与现代医学的协同策略,并总结其临床证据和机制研究进展,旨在为中西医结合治疗心力衰竭提供理论依据和实践参考。
Heart failure is the end-stage of cardiovascular disease,with poor prognosis,and limited treatment options.Theory of“heart-spleen correlation”in traditional Chinese medicine suggests that heart and spleen are interdependent physiologically,and influence each other pathologically,patients with heart failure often experience dysfunction of spleen and stomach,forming a vicious cycle of heart qi deficiency,internal blockade of phlegm and blood stasis.Modern research reveals that patients with heart failure have disrupted intestinal flora,abnormal metabolites,and immune inflammatory activation,which exacerbate myocardial damage.As core strategy of traditional Chinese medicine in the treatment of heart failure,strengthening spleen method can regulate intestinal flora,improve metabolic disorders,inhibit myocardial fibrosis,and exert multi-target synergistic effects through pathways such as tonifying qi,warming yang,regulating qi,and promoting blood circulation.This article starts from theory of“heart-spleen correlation”,systematically elaborates on traditional Chinese medicine pathogenesis logic and point of convergence in modern pathological mechanisms of strengthening spleen method in the intervention of heart failure,proposes individualized application of strengthening spleen method in different types of heart failure syndrome and collaborative strategies with modern medicine,and summarizes clinical evidence and mechanism research progress,the aim is to provide theoretical basis and practical reference for integration of traditional and western medicine in the treatment of heart failure.
