摘要
目的探究老年人体质量指数与抑郁症状的关联,并分析慢性疾病与社会参与的中介关联路径。方法基于2020年中国老年社会追踪调查的横断面数据,选取9882名老年人纳入研究,采用最小二乘法进行线性回归、Sobel检验及似不相关回归,分析体质量指数与老年人抑郁症状的关联、中介效应及异质性。结果相较于正常体质量指数,老年人体重过低时抑郁症状得分更高(P<0.01),超重/肥胖时抑郁症状得分更低(P<0.01)。中介效应模型分析发现慢性疾病数量在其中起遮掩效应,体质量指数提升通过增加慢性疾病数量加重抑郁症状水平,而社会参与在其中起部分中介作用,体质量指数提升通过提高社会参与度改善抑郁症状水平。异质性分析结果显示体质量指数对老年人抑郁症状的影响因居住地区类型和性别的不同而存在差异。结论体质量指数与老年人抑郁症状之间存在关联,且这种关联可能受到慢性疾病与社会参与的中介作用影响,提示在老年人健康管理中应综合考虑体重状况及其心理社会机制。
Objective To explore the association between body mass index and depression symptoms among elderly people in China,and to analyze the mediating association pathway of chronic diseases and social participation in this relationship.Methods Based on cross-sectional data from the 2020 China Elderly Social Tracking Survey,9882 elderly people were selected for the study.Using the least squares method for linear regression,Sobel test and non-correlation regression,the association,mediating effect and heterogeneity between body mass index and depressive symptoms in elderly people were analyzed.Results Compared with the normal body mass index,the depression symptom scores were higher in elderly people when their weight was too low(P<0.01),and lower scores when they were overweight or obese(P<0.01).The mediation effect model analysis found that the number of chronic diseases played a masking effect,and the increase in body mass index aggravated the level of depression symptoms by increasing the number of chronic diseases.Social participation played a partial mediating role,and the increase in body mass index improved the level of depression symptoms by increasing social participation.The heterogeneity analysis results showed that the impact of body mass index on depression symptoms in elderly people varies depending on the type of residential area and gender.Conclusion There is a correlation between body mass index and depression symptoms in elderly people,and this correlation may be mediated by chronic diseases and social participation,suggesting that weight status and its psychosocial mechanisms should be comprehensively considered in elderly people health management.
