摘要
目的探讨基于DeepSeek系统的情境式教学联合多学科团队(multidisciplinary team,MDT)模式在神经内科住院医师规范化培训一年级阶段的多维度、可量化的教学效果。方法选取2025年2月至6月在北京大学人民医院轮转的70名住培第一年学员,随机分为对照组与试验组各35人。对照组实施案例为基础的学习教学模式,试验组则实施基于DeepSeek的情境式模拟教学联合MDT教学模式。通过阶段性理论考试、临床实践操作考核、加利福尼亚批判性思维倾向量表(the California Critical Thinking Disposition Inventory,CCTDI)及教学满意度问卷,综合评估教学成效。结果与对照组相比,试验组理论成绩([82.94±5.55)分vs.(74.31±5.49)分,t=6.44,P<0.001]及临床实践考核成绩([83.63±6.33)分vs.(74.80±5.26)分,t=6.25,P<0.001]均显著提升。CCTDI评估显示,试验组在真相寻找([31.43±6.72)分vs.(24.80±5.60)分,t=4.42,P<0.001]、分析能力([31.29±6.98)分vs.(25.60±4.55)分,t=3.98,P<0.001]等7个维度得分均优于对照组,总得分显著更高([217.20±20.64)分vs.(178.46±16.78)分,t=8.49,P<0.001]。教学满意度调查中,试验组各项目评分均高于对照组,如学习积极性([7.86±1.12分)vs.(6.34±1.04)分,t=5.76,P<0.001]及临床思维培养([7.97±0.97)分vs.(6.17±0.99)分,t=7.53,P<0.001],总得分亦显著提升([54.20±2.61)分vs.(43.80±3.12)分,t=14.20,P<0.001]。结论基于DeepSeek的情境式模拟联合MDT教学模式能有效提升住培初期学员的临床知识掌握、实践操作能力及思维素养,激发其学习积极性与团队协作意识,是优化神经内科住培教学体系、强化初期培训质量的有益尝试。
Objective To evaluate the effectiveness of a DeepSeek-enhanced,scenario-based teaching model combined with a multidisciplinary team approach for first-year neurology residents in standardized training,using multidimensional and quantifiable metrics.Methods From February to June 2025,70 first-year residents rotating at Peking University People's Hospital were randomly assigned to either a control group(n=35)receiving case-based teaching or an intervention group(n=35)receiving DeepSeek-enhanced,scenario-based simulation teaching integrated with multidisciplinary team approach.Outcomes were assessed using periodic theoretical knowledge examinations,clinical skill assessments,the California Critical Thinking Disposition Inventory,and a teaching satisfaction questionnaire.Results The intervention group demonstrated significantly higher scores than the control group in both theoretical knowledge examinations(82.94±5.55 vs.74.31±5.49,t=6.44,P<0.001)and clinical skill assessments(83.63±6.33 vs.74.80±5.26,t=6.25,P<0.001).On the California Critical Thinking Disposition Inventory,the intervention group outperformed the control group across all seven dimensions,including truth-seeking(31.43±6.72 vs.24.80±5.60,t=4.42,P<0.001)and analytical ability(31.29±6.98 vs.25.60±4.55,t=3.98,P<0.001),and achieved a significantly higher total score(217.20±20.64 vs.178.46±16.78,t=8.49,P<0.001).The teaching satisfaction survey also revealed higher scores for the intervention group across all items,including learning motivation improvement(7.86±1.12 vs.6.34±1.04,t=5.76,P<0.001)and clinical thinking cultivation(7.97±0.97 vs.6.17±0.99,t=7.53,P<0.001).The total satisfaction score was also significantly higher for the intervention group(54.20±2.61 vs.43.80±3.12,t=14.20,P<0.001).Conclusions The integration of a DeepSeek-based,scenario-based teaching model with a multidisciplinary team approach can effectively enhance clinical knowledge,practical skills,and critical thinking in early-stage neurology residents.This method can also foster active learning and team collaboration,representing a valuable attempt to optimize the neurology residency training system and to strengthen the quality of early-stage training.
