摘要
目的探讨补充性肠外营养(SPN)联合肠内营养(EN)对消化道肿瘤患者营养状态及治疗耐受性的影响。方法选取2022年6月至2024年6月医院收治的90例消化道肿瘤患者,以随机数字表法将其分为EN组(n=45)和SPN+EN组(n=45)。EN组给予常规营养指导和EN治疗,SPN+EN组在此基础上给予SPN治疗。比较两组的营养状态[总蛋白(TP)、白蛋白(ALB)、血红蛋白(Hb)]、身体状况[体质量指数(BMI)、体脂肪率(BFP)、体水成分(BWC)]、免疫功能[T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)]、治疗耐受性[EN耐受性评分表(ENTS)]及不良反应发生情况。结果SPN+EN组和EN组治疗后的TP、ALB、Hb水平、BMI、BFP、BWC、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于治疗前,且SPN+EN组高于EN组(P<0.05)。SPN+EN组治疗3、6周后的ENTS评分低于EN组(P<0.05)。SPN+EN组的不良反应总发生率低于EN组(P<0.05)。结论SPN联合EN可改善消化道肿瘤患者的营养状态、身体状况、免疫功能,增强治疗耐受性,减少不良反应发生情况,值得临床推广。
Objective To investigate the effects of supplemental parenteral nutrition(SPN)combined with enteral nutrition(EN)on nutritional status and treatment tolerance in patients with gastrointestinal cancer.Methods A total of 90 patients with gastrointestinal cancer admitted in hospital from June 2022 to June 2024 were selected and divided into EN group(n=45)and SPN+EN group(n=45)by random number table method.The EN group was given routine nutritional guidance and EN treatment,and the SPN+EN group was given SPN treatment on this basis.The nutritional status[total protein(TP),albumin(ALB),hemoglobin(Hb)],physical condition[body mass index(BMI),body fat percentage(BFP),body water composition(BWC)],immune function[T lymphocyte subsets CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)],treatment tolerance[Enteral Nutrition Tolerance Score(ENTS)]and adverse reactions were compared between the two groups.Results The TP,ALB,Hb levels,BMI,BFP,BWC,CD3^(+),CD4^(+),CD4^(+)/CD8+in the SPN+EN group and the EN group after treatment were higher than those before treatment,and those of the SPN+EN group were higher than the EN group(P<0.05).The ENTS score of the SPN+EN group was lower than that of the EN group after 3 and 6 weeks of treatment(P<0.05).The total incidence of adverse reactions in the SPN+EN group was lower than that in the EN group(P<0.05).Conclusion SPN combined with EN can improve the nutritional status,physical condition and immune function of patients with gastrointestinal cancer,enhance the tolerance of treatment and reduce adverse reactions.It is worthy of clinical promotion.
