摘要
目的探讨基于家庭为中心的个案管理模式对肝动脉化疗栓塞术(Transcatheter Arterial Chemoembolization,TACE)围手术期肝癌患者心理状态、生存质量、术后并发症、住院时间、住院总费用及护理满意度的改善效果。方法通过便利抽样法,选取我院2023年1月-2024年11月收治的76例行TACE治疗的肝癌患者,随机分为观察组与对照组,每组38例。对照组实施常规护理,观察组在常规护理的基础上实施基于家庭为中心的个案管理模式。观察两组患者的心理状态、生存质量、术后并发症、住院时间、住院总费用及护理满意度。结果干预后,观察组的生存质量(FACT-L)评分及护理满意度均高于对照组(P<0.05),观察组患者的HADS-A、HADS-D评分,术后并发症发生率,住院时间以及住院总费用均低于对照组(P<0.05)。结论基于家庭为中心的个案管理模式能显著改善TACE围手术期肝癌患者焦虑抑郁水平,提高患者生存质量和护理满意度,降低术后并发症发生率,缩短住院时间并减少住院总费用。
Objective This study aims to explore the improvement effects of family-centered case management model on psychological status,quality of life,postoperative complications,length of hospital stay,total hospitalization cost and nursing satisfaction of liver cancer patients undergoing transcatheter arterial chemoembolization(TACE)during the perioperative period.Methods A total of 76 patients with liver cancer who underwent transcatheter arterial chemoembolization(TACE)in our hospital from January 2023 to November 2024 were selected by convenience sampling method,and then randomly divided into the observation group and the control group by block randomization,with 38 cases in each group.The control group was given routine nursing care,while the observation group was applied family-centered case management model on the basis of routine nursing care.The psychological status,quality of life,postoperative complications,length of hospital stay,total hospitalization cost and nursing satisfaction of the two groups were observed and compared.Results After the intervention,the scores of quality of life(FACT-L)and nursing satisfaction in the observation group were significantly higher than those in the control group(P<0.05).Meanwhile,the HADS-A/HADS-D scores,incidence of postoperative complications,length of hospital stay and total hospitalization cost in the observation group were all lower than those in the control group(P<0.05).Conclusion The family-centered case management model can significantly alleviate the anxiety and depression levels of liver cancer patients during the perioperative period of transcatheter arterial chemoembolization(TACE),improve their quality of life and nursing satisfaction,reduce the incidence of postoperative complications,shorten the length of hospital stay and cut down the total hospitalization cost.
