摘要
目的探讨护士主导下认知和康复运动训练(CRGTN)对直肠癌术后患者免疫功能及营养状况的影响。方法选取2017年1月至2019年12月间营口市中心医院行直肠癌根治手术的156例直肠癌患者作为研究对象,按照随机数表法分为观察组和对照组,每组78例。观察组患者给予CRGTN,对照组患者给予围术期常规护理。比较两组患者临床康复指标,及术前1 d、术后7 d的免疫指标和营养指标。结果观察组患者肛门排气时间、肠道恢复时间、尿管留置时间、开始半流质时间、下床活动时间和住院时间均短于对照组患者,差异均有统计学意义(均P<0.05)。重复测量方差分析显示,时点效应可以影响免疫球蛋白G(IgG)、IgA、IgM、前清蛋白、清蛋白和总蛋白水平的变化,干预方式可以影响IgG、IgA、IgM、前清蛋白、清蛋白和总蛋白水平的变化(均P<0.05)。且时点和治疗方式之间存在着交互效应(P<0.05)。结论CRGTN可显著缩短直肠癌患者术后康复时间,增强免疫功能,改善营养状况。
Objective To explore the effects of cognitive and rehabilitation exercises on postoperative immune function and nutritional status in patients with rectal cancer.Methods From January 2017 to December 2019,156 patients undergoing radical resection of rectal cancer at Yingkou Central Hospital were selected.Using random number table method,they were randomly divided into an observation group and a control group with 78 patients in each group.The observation group were given cognitive and rehabilitation exercise training under the guidance of nurses,while the control group were given conventional perioperative care.The clinical rehabilitation indexes and immune indices and nutrition status were compared between the two groups at 1 d before the surgery and 7 d after the surgery.Results The time to anal exhaust,time to intestinal recovery,length of urinary catheter stay,time to semifluid initiation,time to out-of-bed activity and length of hospital stay were significantly shorter in the observation group than in the control group(all P<0.05).Two-Way Repeated Measures ANOVA showed that point-in-time effect and intervention methods can significantly influence changes in immunoglobulin G(IgG),IgA,IgM,prealbumin,albumin,and total protein levels(all P<0.05),and there was a significant interaction between time points and treatment methods(all P<0.05).Conclusion Under the guidance of nurses,cognitive and rehabilitation training can significantly shorten postoperative rehabilitation time,enhance immune function and improve nutritional status of rectal cancer patients.
