单次微波消融治疗甲状腺良性结节的效果观察

摘要 目的:观察单次微波消融治疗甲状腺良性结节的效果。方法:选取2023年1月—2024年12月上海交通大学医学院附属瑞金医院收治的甲状腺良性结节患者120例,采用随机数字表法分为对照组与观察组,各60例。对照组行甲状腺切除术,观察组行单次微波消融治疗。对比两组治疗效果。结果:观察组手术时间及住院时间短于对照组(P<0.001),术中出血量少于对照组(P<0.001);术后第3天,观察组白细胞介素-6、C反应蛋白、肿瘤坏死因子-α水平比对照组低(P<0.001),游离三碘甲状腺原氨酸、游离甲状腺素水平比对照组高(P<0.001),促甲状腺激素水平比对照组低(P<0.001);术后,观察组视觉模拟评分法评分比对照组低(P<0.001);观察组并发症总发生率低于对照组(P=0.013)。结论:单次微波消融治疗甲状腺良性结节的效果显著,能够缩短手术时间及住院时间,减少术中出血量,改善甲状腺功能,减轻术后炎症及疼痛,降低并发症发生率。 Objective:To observe the effect of single microwave ablation therapy for benign thyroid nodules.Methods:A total of 120 patients with benign thyroid nodules admitted to Ruijin Hospital of Shanghai Jiaotong University School of Medicine from January 2023 to December 2024 were selected and divided into the control group and the observation group according to the random number table method,with 60 cases in each group.The control group was treated with thyroidectomy,and the observation group was treated with single microwave ablation.The therapeutic effects of the two groups were compared.Results:The operation time and length of hospital stay in the observation group were shorter than those in the control group(P<0.001),and the intraoperative blood loss in the observation group was less than that in the control group(P<0.001).On the 3 d after surgery,the levels of interleukin-6,C-reactive protein,and tumor necrosis factor-αin the observation group were lower than those in the control group(P<0.001),the levels of free triiodothyronine and free thyroxine in the observation group were higher than those in the control group(P<0.001),and the level of thyroid stimulating hormone in the observation group was lower than that in the control group(P<0.001).After surgery,the visual analogue scale score in the observation group was lower than that in the control group(P<0.001).The total incidence of complications in the observation group was lower than that in the control group(P=0.013).Conclusion:Single microwave ablation achieves significant effects in the treatment of benign thyroid nodules,and can shorten the operation time and length of hospital stay,reduce intraoperative blood loss,improve thyroid function,relieve postoperative inflammation and pain,and reduce the incidence of complications.
