摘要
目的:对比不同强度负压吸引用于甲状腺癌术后乳糜漏的效果。方法:选取2021年1月—2024年5月于眉山市人民医院行甲状腺切除术与颈部淋巴结清扫术的甲状腺癌患者90例,采用随机数字表法分为低压组(-25.4~-10.2 kPa)、中压组(-42.8~-25.5 kPa)及高压组(-80.9~-42.9 kPa),各30例。比较三组乳糜液引流量、引流管留置时间、不良反应发生情况。结果:高压组引流8、16、24 h乳糜液引流量高于中压组及低压组(P<0.05);中压组各时间乳糜液引流量高于低压组(P<0.001)。中压组引流管留置时间短于低压组、高压组(P<0.05)。高压组不良反应发生率高于中压组及低压组(P=0.022)。结论:-42.8~-25.5 kPa负压吸引强度用于甲状腺癌颈部淋巴结清扫术后并发乳糜漏患者的效果较好,可在有效引流的同时缩短引流管留置时间,且安全性良好。
Objective:To compare the effects of different intensity negative pressure suction on the occurrence of chyle leakage after thyroid cancer surgery.Methods:A total of ninety patients with thyroid cancer who underwent thyroidectomy and cervical lymph node dissection at Meishan People's Hospital from January 2021 to May 2024 were selected.The subjects were divided into the low-pressure group(-25.4 to-10.2 kPa),the medium-pressure group(-42.8 to-25.5 kPa),and the high-pressure group(-80.9 to-42.9 kPa)using the random number table method,with thirty cases in each group.The volume of chyle drainage,the duration of drainage tube insertion,and the occurrence of adverse reactions among the three groups were compared.Results:The volume of chyle drainage at 8,16,and 24 hours in the high-pressure group was higher than that in the medium-pressure group and the low-pressure group(P<0.05).The volume of chyle drainage in the medium-pressure group was higher than that in the low-pressure group at each time point(P<0.001).The retention time of the drainage tube in the medium-pressure group was shorter than that in the low-pressure group and the high-pressure group(P<0.05).The incidence of adverse reactions in the highpressure group was higher than that in the medium-pressure group and the low-pressure group(P=0.022).Conclusion:A negative pressure suction intensity of-42.8 to-25.5 kPa is effective for patients with chyle leakage after thyroid cancer neck lymph node dissection.It can achieve effective drainage while shortening the retention time of the drainage tube,and has good safety.
