摘要
根据医改新形势下公立医院高质量发展的趋势,运用SWOT分析医院在内、外部及宏观环境中存在的问题,结合医院实际情况,针对医疗工作中的管理缺陷、医疗隐患、质控薄弱环节及工作中的重点难点,研究探索基于全面质量管理(total quality management,TQM)模型构建的应用可行性及适用性。本研究借鉴国内外医院TQM管理模式经验方法,从4个维度包括战略基础、评价体系、持续改进及信息化系统等构建模型体系框架,利用信息化及大数据AI等协同对患者诊疗前、诊疗中及诊疗后全流程进行实时动态监控,及时反馈,落实整改。医院成功实施TQM专科能力指数、医疗质量指数、患者满意度评价等明显提升,实施前后成效显著。公立医院实施TQM各维度对组织绩效产生积极的影响,可为基层医院实施全面质量管理借鉴应用提供参考。
Under the new healthcare reform and the trend toward high-quality development in public hospitals,this study employed a SWOT analysis to identify issues within the hospital’s internal and external macro-environment.Based on the actual conditions of the hospital,we explored the feasibility and applicability of constructing a total quality management(TQM)model to address management deficiencies,latent clinical risks,weaknesses in quality control,and key operational challenges in healthcare delivery.Drawing on domestic and international hospital TQM practices,a model framework was built with four dimensions:strategic foundation,evaluation system,continuous improvement,and information systems.Utilizing information technology,big data,and AI,the model enabled real-time dynamic monitoring and closed-loop feedback for correction throughout the entire patient care process—before,during,and after treatment.After implementation,the hospital demonstrated significant improvements in TQM-based specialty competency indices,medical quality metrics,and patient satisfaction ratings,with notable outcomes before and after implementation.The application of TQM across these dimensions positively influenced organizational performance in the public hospital setting,providing a reference for primary hospitals to implement systematic quality management.
