摘要
目的:比较间断垂直钢丝缝合与钢丝环扎2种术式对移位型髌骨下极骨折术后疗效的影响。方法:选取2020年1月—2023年1月于新疆生产建设兵团第三师总医院骨科治疗移位型髌骨下级骨折的116例患者作为研究对象,分别采用2种不同手术方式固定,A组行间断垂直钢丝缝合共66例;B组行钢丝“O”型环扎共50例。分别记录并比较两组患者年龄、性别、体质量指数(BMI)、基础疾病、美国麻醉医师协会(ASA)评分、骨折类型和吸烟情况,并对术后12个月两组患者并发症、患侧膝关节活动度、膝关节功能进行随访。结果:A组患者术后膝关节活动度明显大于B组,差异有统计学意义(P<0.05);A组患者术后膝关节Bostman评分明显优于B组,差异有统计学意义(P<0.05);两组患者手术相关指标和并发症发生情况,差异均无统计学意义(P>0.05)。结论:与钢丝“O”型环扎技术相比,采用间断垂直钢丝缝合技术治疗的髌骨下极骨折患者膝关节活动度和膝关节Bostman评分较好,且两组患者具有相似的手术安全性,间断垂直钢丝缝合技术应被考虑作为治疗髌骨下极骨折的替代疗法。
Objective:To compare the postoperative outcomes of displaced inferior patellar fractures treated with intermittent vertical wire suturing versus wire ring fixation.Methods:From January 2020 to January 2023,116 cases of displaced infrapatellar fractures treated in the Orthopedic Department of the Third Division General Hospital of the Xinjiang Production and Construction Corps were selected,with two distinct surgical fixation techniques applied.Group A underwent interrupted vertical wire suturing(n=66),while Group B received wire"O"-shaped circumferential fixation(n=50).The age,gender,body mass index(BMI),underlying medical conditions,American Society of Anesthesiologists(ASA)physical status classification,fracture type,and smoking status of the two patient groups were recorded and compared.Additionally,complications,range of motion of the affected knee,and knee function were followed up and compared between the two groups at 12 months postoperatively.Results:The knee joint range of motion was significantly greater in Group A patients than in Group B patients after surgery,with a statistically significant difference(P<0.05).Postoperative knee joint Bostrom scores were significantly higher in Group A than in Group B,with a statistically significant difference(P<0.05).There was no statistically significant difference between the two groups in terms of surgery-related indicators and complication rates(P>0.05).Conclusion:Compared with steel wire"O"-ring fixation technique,patients treated with interrupted vertical steel wire suturing for infrapatellar fractures demonstrated superior knee range of motion and Bostrom knee function scores at 12-month follow-up.Both groups exhibited comparable surgical safety.Interrupted vertical steel wire suturing should be considered as an alternative treatment for infrapatellar fractures.
