摘要
小儿鹅口疮由白色念珠菌机会性感染所引发,复发率高,并有进展为侵袭性真菌感染的可能,从而威胁生命。现代研究揭示了其发病机制主要包括免疫功能低下与口腔微生态失衡两方面,其中涉及生理特点、药物使用等诸多因素;而中医认为其核心病机在于热邪熏蒸口舌,并强调了以心、脾为主的脏腑功能失调与口腔病变的关联性。治疗上,西医多采用传统抗真菌药物,虽可有效抑制真菌增殖,但存在耐药性及较高的复发率。随着研究的不断深入,益生菌、免疫调节剂等被证实具有辅助抗真菌作用,为用药提供了新选择。中医治疗则注重应用“辨证施治”思维,通过中药制剂外用或内服等方法取得了良好效果。中西医结合疗法作为新模式,通过整合西药的快速抗真菌作用与中药的系统调理作用发挥协同效应,达到标本同治的目的,疗效显著,为目前研究热点。该文通过整理近年来国内外相关文献,探讨了小儿鹅口疮的病因、发病机制、中西医治疗策略及最新研究进展,以期为小儿鹅口疮的治疗提供更为科学合理的用药建议。
Pediatric oral thrush,caused by opportunistic infection with Candida albicans,demonstrates high recurrence rate and can progress to invasive fungal infections,which is potentially life threaten.Modern studies attribute its pathogenesis to two primary mechanisms:immunodeficiency and disruption of oral microbial homeostasis,both of which are affected by physiological conditions,medicine use,and other factors.Traditional Chinese medicine(TCM)identifies the core pathological mechanism as the“accumulation of pathogenic heat”in the oral cavity and tongue,linking the condition to systemic dysfunction of organs,particularly the heart and spleen.Conventional Western therapies rely on antifungal agents to inhibit fungal growth,yet face challenges such as drug resistance and frequent relapse.Emerging adjunctive strategies,including probiotics and immunomodulators,demonstrate synergistic antifungal effects,broadening treatment options.TCM emphasizes a syndrome differentiation-based approach,utilizing topical or oral herbal formulations to restore balance,showing a significant clinical effect.Integration of Traditional Chinese medicine and Western medicine,a novel model combining the rapid antifungal action of Western medicine with TCM's holistic regulatory effects,has gained prominence.This dual approach targets both acute symptoms and underlying imbalances,achieving remarkable synergistic outcomes and reducing recurrence.This article reviews recent literature on pediatric oral thrush,analyzing its etiology,pathogenesis mechanism,and therapeutic strategies from perspectives of both Western medicine and traditional TCM,aiming to provide evidence-based,personalized management strategies for pediatric oral thrush.
