摘要
总结刘丽坤教授运用麻黄升麻汤治疗鼻咽癌的经验。认为本病以气阴两虚为本,痰瘀热毒为标,治疗重点在于益气养阴扶正的同时,兼顾祛痰、瘀、热、毒之病理因素。根据临床经验认为麻黄升麻汤寒热并用、补虚清实之功正契合鼻咽癌之病机。临证以麻黄升麻汤为基础治疗鼻咽癌,依据患者虚实之侧重,调整方中清热与温中药物比例,根据患者邪气郁闭之程度,巧用麻黄剂量。依据临床症状,分经论治,各药用量随证加减,共达宣通郁闭、寒热同调、补虚清实、标本同治之功效。并附验案1则。
This article summarizes Professor LIU Likun’s experience in treating nasopharyngeal carcinoma with Mahuang Shengma Decoction(麻黄升麻汤).It is believed that this disease is characterized by deficiency of both qi and yin as the root cause,with phlegm,blood stasis,heat and toxins as the manifestations.The key to treatment lies in tonifying qi,nourishing yin and strengthening the body’s resistance,while addressing the pathological factors of phlegm,blood stasis,heat,and toxin.Based on clinical experience,it is believed that the Mahuang Shengma Decoction,which combines cold and heat and has the functions of tonifying deficiency and clearing excess,precisely aligns with the pathogenesis of nasopharyngeal carcinoma.In clinical practice,Mahuang Shengma Decoction is used as the basis for treating nasopharyngeal carcinoma.According to the patient’s condition on deficiency and excess,the proportion of heat-clearing and warming medicinals in the formula is adjusted.Based on the degree of the patient’s pathogenic qi stagnation,the dosage of Mahuang(Ephedrae Herba)is skillfully used.Based on clinical symptoms,channels-based differen⁃tiation and treatment is given,and the dosage of each medicinal is adjusted according to the syndrome to achieve the effects of promoting the circulation,relieving obstruction and depression,and regulating cold and heat simultaneously,tonifying deficiency and clearing excess,and treating both symptoms and root causes.One test case is attached.
