摘要
目的研究消囊调经汤对肥胖型多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者性激素水平及卵巢间质血流的影响。方法选取88例肥胖型PCOS患者,采用随机数字表法进行分组,对照组(n=44)给予炔雌醇环丙孕酮片常规治疗,研究组(n=44)在对照组基础上增加消囊调经汤治疗。治疗3个月后,比较2组患者中医证候积分、性激素指标、脂代谢水平、卵巢间质血流指标、排卵率和妊娠率以及不良反应。结果治疗后,2组患者中医证候积分均显著降低(P均<0.05),且研究组显著低于对照组(P<0.05)。2组患者黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle-stimulating hormone,FSH)、睾酮(testosterone,T)水平均显著降低(P均<0.05),雌二醇(estradiol,E 2)水平均显著升高(P均<0.05),且研究组LH、FSH、T水平均显著低于对照组(P均<0.05),E 2水平显著高于对照组(P<0.05)。2组患者子宫动脉搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)均显著降低(P均<0.05),且研究组显著低于对照组(P均<0.05)。2组患者高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)水平均显著升高,甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、总胆固醇(total cholesterol,TC)水平均显著降低(P均<0.05),且研究组HDL-C水平显著高于对照组(P<0.05),LDL-C、TG、TC水平均显著低于对照组(P均<0.05)。治疗后,研究组排卵率为90.91%,显著高于对照组的68.18%(P<0.05);研究组妊娠率为29.55%,显著高于对照组的11.36%(P<0.05)。2组不良反应总发生率组间比较,差异无统计学意义(P>0.05)。结论消囊调经汤能够有效改善肥胖型PCOS患者卵巢间质血流指标,调节脂代谢和性激素水平,提高排卵率和妊娠率,安全性高。
Objective To study the effects of Xiaonang Tiaojing decoction on the sex hormone levels and ovarian interstitial blood flow in obese patients with polycystic ovary syndrome(PCOS).Methods Eighty-eight obese patients with PCOS were selected and randomly divided into two groups using a random number table method.The control group(n=44)received conventional treatment with ethinylestradiol and cyproterone acetate tablets,while the study group(n=44)received additional treatment with Xiaonang Tiaojing decoction on the basis of the control group.After 3 months of treatment,The TCM syndrome scores,sex hormone indicators,lipid metabolism levels,ovarian interstitial blood flow indicators,ovulation rate,pregnancy rate,and adverse reactions of the two groups of patients were compared.Results After treatment,the TCM syndrome scores of both groups were significantly reduced(all P<0.05),and the study group was significantly lower than the control group(P<0.05).The levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH)and testosterone(T)in both groups of patients were significantly reduced(all P<0.05),while the levels of estradiol(E 2)were significantly increased(all P<0.05).The levels of LH,FSH and T in the study group were significantly lower than those in the control group(all P<0.05),and the level of E 2 was significantly higher than that in the control group(P<0.05).The levels of uterine artery pulsatility index(PI)and resistance index(RI)were significantly reduced in both groups of patients(all P<0.05),and the study group was significantly lower than the control group(all P<0.05).The levels of high-density lipoprotein cholesterol(HDL-C)were significantly increased in both groups of patients(all P<0.05),while the levels of triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)and total cholesterol(TC)were significantly decreased(all P<0.05).The HDL-C level in the study group was significantly higher than that in the control group(P<0.05),while the levels of LDL-C,TG and TC were significantly lower than those in the control group(all P<0.05).After treatment,the ovulation rate of the study group was 90.91%,which was significantly higher than the control group's 68.18%(P<0.05).The pregnancy rate of the study group was 29.55%,which was significantly higher than the control group's 11.36%(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Xiaonang Tiaojing decoction could effectively improve the ovarian interstitial blood flow indicators in obese PCOS patients,regulate lipid metabolism and sex hormone levels,increase ovulation and pregnancy rates,and had high safety.
