摘要
慢性再生障碍性贫血(chronic aplastic anemia,CAA)是以全血细胞减少、贫血、出血和感染为典型特征的血液科疾病,对于CAA障的治疗西医学尚无特效疗法,而中医治疗具有天然优势。本文概括总结团队基于“固本培元”理论辨治CAA的临床经验,认为CAA的病机关键在于元气亏虚,根植于脾肾不足。肾乃元气之本,命门火衰,元气亏虚;脾可充养元气,中州虚衰,阴火乘位,元气亏虚;元气亏虚,进而导致髓海瘀毒阻络。治法以固本培元为核心,具体为:温补命门,填补下焦以保元;温养气血,培补中焦以养元;温化痰瘀,疏肝解毒以护元。同时,治疗CAA临证有道,需注意脏虚有异,治有先后;审机为先,以果为据;性味合化,久病徐图,以期为CAA的治疗及理论研究提供借鉴。
Chronic aplastic anemia(CAA)is a hematological disorder characterized by pancytopenia,anemia,bleeding,and infections.While modern medicine lacks specific effective therapies for CAA,Traditional Chinese Medicine(TCM)offers a natural advantage in treatment.This article summarizes the clinical experience of our team in treating CAA based on the theory of"strengthening the foundation and cultivating the original qi",and believes that the pathogenesis of CAA lies in the deficiency of qi,which is rooted in the deficiency of spleen and kidney.The kidney is the source of the original qi.With the decline of life gate fire,the original qi becomes deficient.The spleen nourishes the original qi.However,when the middle jiao is weak,yin fire takes over,leading to deficiency of the original qi.Deficiency of the original qi then causes toxic stasis obstructing the marrow network.The therapeutic strategy centers around consolidating the fundamental and cultivating the original qi.To be specific,the treatment methods include warming and replenishing the gate fire and filling the lower jiao to preserve the original qi,warming and nourishing blood and qi while cultivating the middle jiao to nurture the original qi,warming and resolving phlegm and stasis and soothing the liver and removing toxins to protect the original qi.This approach aims to provide reference for the treatment and theoretical research of CAA.
