摘要
神经病理性疼痛(neuropathic pain, NP)是脊髓损伤(spinal cord injury, SCI)后常见的并发症,其发病率高、机制复杂且药物治疗效果常不理想。近年来,神经调控技术在SCI导致的NP中展现出良好的应用前景。该综述从神经炎症及胶质细胞活化、神经递质失调、离子通道表达异常及神经营养因子失调等方面系统梳理了SCI后NP的病理生理机制,并重点介绍了重复经颅磁刺激、经颅直流电刺激、重复脊髓磁刺激等非侵入技术和高频脊髓电刺激及脑深部电刺激等侵入性技术在SCI后NP中的作用机制及临床应用现状。旨在整合当前研究进展,为深入理解SCI后NP的发生机制及临床合理应用神经调控技术提供参考。
Neuropathic pain(NP)is a common complication following spinal cord injury(SCI),characterized by high incidence,complex mechanisms,and often suboptimal response to pharmacological treatment.In recent years,neuromodulation techniques have demonstrated promising potential for managing NP associated with SCI.This review systematically outlines the pathophysiological mechanisms of NP after SCI,focusing on key factors including neuroinflammation and glial cel activation,dysregulation of neurotransmitter,abnormal ion channel expression,and neurotrophic factor imbalance.Furthermore,it focuses on the mechanisms of action and current clinical applications of both non-invasive neuromodulation techniques—such as repetitive transcranial magnetic stimulation,transcranial direct current stimulation,and repetitive trans-spinal magnetic stimulation,and invasive techniques,including high-frequency spinal cord stimulation and deep brain stimulation.The aim of this review is to synthesize current research advances to enhance the understanding of NP mechanisms post-SCI and to support the rational clinical application of neuromodulation strategies.
