摘要
目的:运用数据挖掘技术剖析针灸治疗卒中后上肢运动障碍的选穴规律。方法:在中国知网期刊全文数据库、维普中文科技期刊数据库、万方学术期刊全文数据库及中国生物医学文献数据库中全面检索建库至2025年1月22日收录的有关针灸治疗卒中后上肢运动障碍的文献,借助Microsoft Excel 2019构建数据库,针对腧穴实施描述性分析,利用SPSS Modeler 18.0开展关联规则分析,运用Cytoscape 3.10.3构建高频腧穴共现网络图,运用SPSS Statistics 26.0对腧穴进行层次聚类分析并绘制树状图。结果:总计纳入124篇相关文献,获得完整腧穴处方124首,涉及112个腧穴,腧穴的总使用频次达890次,使用频次居前3位的腧穴分别为肩髃穴、曲池穴、合谷穴。在腧穴归经方面,使用频次最高的经脉为手阳明大肠经;在腧穴关联规则方面,曲池穴–肩髃穴、合谷穴–曲池穴、外关穴–肩髃穴属于核心腧穴组合;聚类分析显示了阴经和阳经两个有效聚类群。结论:针灸治疗卒中后上肢运动障碍时所选穴位以阳经腧穴为主,常以肩髃穴、曲池穴、合谷穴为核心穴位,取穴集中在手阳明大肠经,采用“从阴引阳,从阳引阴”的治疗思路,为现代中医临床治疗卒中后上肢运动障碍提供参考。
Objective:To analyze the acupoint selection rules of acupuncture and moxibustion on upper limb motor disorders after stroke based on data mining.Methods:Relevant studies on treating upper limb motor disorders after stroke by acupuncture and moxibustion in CNKI,VIP,Wanfang,and CBM databases from the inception to January 22,2025 were retrieved to establish a prescription database by the Microsoft Excel 2019.A descriptive analysis of acupoints was conducted.The IBM SPSS Modeler 18.0 was used for association rule analysis of acupoints.The Cytoscape 3.10.3 was utilized for graphing the co-occurrence network of high-frequency acupoints.The SPSS Statistics 26.0 was used to perform hierarchical cluster analysis of acupoints,and a tree diagram was plotted.Results:A total of 124 relevant articles were included,and 124 complete acupoint prescriptions were obtained,involving 112 acupoints,with a cumulative frequency of 890 times.The top three acupoints in terms of usage frequency were Jianyu(LI15),Quchi(LI11),Hegu(LI4).In terms of meridian tropism of selected acupoints,the most frequently used meridian was Hand Yangming(阳明)Large Intestine Meridian.In terms of the acupoint association rules,Quchi-Jianyu,Hegu-Quchi,Waiguan(SJ5)-Jianyu were the core acupoint combinations.The cluster analysis revealed two effective clustering groups,namely Yin(阴)meridian clustering group and Yang(阳)meridian clustering group.Conclusion:The selected acupoints in the treatment of upper limb motor disorders after stroke are mainly Yang meridian acupoints,often with Jianyu,Quchi,and Hegu acupoints as the core.The selected acupoints are mainly on Hand Yangming Large Intestine Meridian,and the treatment idea is“drawing Yin from Yang,and drawing Yang from Yin”,which provides reference for the clinical treatment in modern Chinese medicine.
