摘要
目的探讨孕早期空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycated hemoglobin A1c,HbA1c)及空腹胰岛素(fasting insulin,FINS)水平联合检测对妊娠期糖尿病(gestational diabetes mellitus,GDM)的诊断价值。方法回顾性选取2023年1月—2024年12月福建省三明市宁化县妇幼保健院产科建档的120例孕妇的临床资料,其中60例GDM孕妇为观察组,60例正常孕妇为对照组。所有孕妇于孕8~14周进行FPG、HbA1c及FINS检测,计算胰岛素抵抗指数。通过受试者操作特征曲线分析单项及联合检测的诊断效能。结果观察组FPG、HbA1c、FINS水平及胰岛素抵抗指数均高于对照组,差异均有统计学意义(P均<0.05)。联合检测的曲线下面积为0.943,灵敏度为91.67%,特异度为90.00%,高于单项检测。结论孕早期FPG、HbA1c及FINS联合检测可提高GDM的早期诊断效能,为临床早期干预提供重要依据。
Objective To explore the diagnostic value of combined detection of fasting plasma glucose(FPG),glycated hemoglobin(HbA1c)and fasting insulin(FINS)levels in early pregnancy for gestational diabetes(GDM).Methods The clinical data of 120 pregnant women who were filed in the Department of Obstetrics,Maternal and Child Health Hospital of Ninghua County,Sanming City,Fujian Province from January 2023 to December 2024 were retrospectively selected.Among them,60 pregnant women with GDM were the observation group and 60 normal pregnant women were the control group.FPG,HbA1c and FINS were detected in all pregnant women at 8-14 weeks of gestation,and the homeostatic model assessment of homeostatic model assessment of insulin resistance was calculated.The diagnostic efficacy of single and combined detection was analyzed by receiver operating characteristic curve.Results The levels of FPG,HbA1c,FINS and homeostatic model assessment of insulin resistance in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The area under the curve of combined detection was 0.943,the sensitivity was 91.67%,and the specificity was 90.00%,which was higher than that of single detection.Conclusion The combined detection of FPG,HbA1c,and FINS in early pregnancy can significantly improve the early diagnostic efficacy of GDM,providing important evidence for early clinical intervention.
