摘要
目的探讨麦克尔憩室(Meckel's diverticulum,MD)的临床表现、诊断、治疗及预后。方法通过分析1例鱼刺致麦克尔憩室穿孔的临床资料,参考国内外关于麦克尔憩室相关文献进行探讨。结果本例患者入院后完善相关术前检查,行腹腔镜探查后,继而中转开腹探查确诊鱼刺致麦克尔憩室穿孔,行单纯憩室切除术,患者术后痊愈,随访恢复可。结论麦克尔憩室是回肠末端发育异常所致,鱼刺致麦克尔憩室穿孔在临床上罕见,麦克尔憩室在术前诊断具有挑战性,无症状麦克尔憩室是否需要手术仍存争议,有症状麦克尔憩室需行个体化手术治疗。
Objective To explore the clinical presentation,diagnosis,treatment,and prognosis of Meckel's diverticulum(MD).Methods The study analyzed the clinical data of a case of Meckel's diverticulum perforation caused by a fish bone and discussed it in combination with relevant literature on Meckel's diverticulum from domestic and international sources.Results After admission,the patient underwent comprehensive preoperative examinations.Laparoscopic exploration followed by conversion to open abdominal exploration confirmed the diagnosis of Meckel's diverticulum perforation caused by a fish bone.A simple diverticulectomy was performed.The patient recovered well postoperatively,and follow-up showed satisfactory recovery.Conclusion Meckel's diverticulum results from abnormal development of the distal ileum.Perforation of Meckel's diverticulum caused by a fish bone is rare in clinical practice.Preoperative diagnosis of Meckel's diverticulum is challenging.Whether asymptomatic Meckel's diverticulum requires surgical intervention remains controversial,while symptomatic cases necessitate individualized surgical treatment.
