摘要
目的:比较消化道异物磁珠致小儿消化道穿孔不同治疗方法的疗效。方法:选取2016年1月—2023年12月河南省儿童医院收治的126例消化道异物磁珠致消化道穿孔患儿作为研究对象,按照手术方式不同将患儿分为开腹组和腔镜组,每组各63例。开腹组采用开腹手术治疗,腔镜组采用腹腔镜手术治疗,比较两组患儿的临床疗效、术中指标、术后指标、住院时间、治疗费用及术后并发症发生率。结果:与开腹组比较,腔镜组手术切口更小,手术时间更短,术中出血量更少(P<0.05)。与开腹组比较,腔镜组术后肛门首次排气时间、首次进食时间和首次下床活动时间均更短(P<0.05)。与开腹组比较,腔镜组住院时间更短(P<0.05);两组治疗费用差异不显著(P>0.05)。腔镜组术后并发症发生率为3.17%,显著低于开腹组的14.29%(P<0.05)。结论:在消化道异物磁珠致小儿消化道穿孔的治疗中,与传统开腹手术比较,采用腹腔镜手术治疗可有效减轻患儿创伤,降低术后并发症发生率,促进患儿康复,且腹腔镜手术并未大幅增加治疗费用,值得临床推广。
Objective:To compare the efficacy of different treatment methods for pediatric gastrointestinal perforation caused by magnetic beads as digestive tract foreign bodies.Methods:A total of 126 children with gastrointestinal perforation caused by magnetic beads admitted to Henan Children's Hospital from January 2016 to December 2023 were selected as the study subjects.According to the surgical approach,the children were divided into an open surgery group and a laparoscopic group,with 63 cases in each group.The open surgery group underwent laparotomy,while the laparoscopic group underwent laparoscopic surgery.The clinical efficacy,intraoperative indicators,postoperative indicators,hospitalization duration,treatment costs,and incidence of postoperative complications were compared between the two groups.Results:Compared with the open surgery group,the laparoscopic group had a smaller surgical incision,shorter operation time,and less intraoperative blood loss(P<0.05).The laparoscopic group also exhibited shorter times to first postoperative anal exhaust,first oral intake,and first ambulation(P<0.05).Additionally,the laparoscopic group had a shorter hospitalization duration(P<0.05),while there was no significant difference in treatment costs between the two groups(P>0.05).The incidence of postoperative complications in the laparoscopic group was 3.17%,significantly lower than the 14.29%in the open surgery group(P<0.05).Conclusion:In the treatment of pediatric gastrointestinal perforation caused by magnetic beads,laparoscopic surgery is more effective than traditional laparotomy in reducing trauma,lowering the incidence of postoperative complications,and promoting recovery,without significantly increasing treatment costs.Thus,it is worthy of clinical promotion.
