摘要
目的探讨决策冲突在晚期癌症患者代理决策者社会支持与决策疲劳间的中介作用。方法采用便利抽样法,于2024年3—7月选取某医科大学附属肿瘤医院接受治疗的晚期癌症患者代理决策者328名为研究对象,采用一般资料调查表、决策疲劳量表、决策冲突量表及社会支持评定量表进行调查。运用AMOS 26.0构建结构方程模型,并采用Bootstrap法对中介效应模型进行检验。结果本组晚期癌症患者代理决策者决策疲劳总分为(15.03±2.16)分,决策冲突总分为(28.21±4.96)分,社会支持总分为(36.51±8.81)分。本组晚期癌症患者代理决策者决策疲劳总分与决策冲突总分呈正相关(r=0.330,P<0.01),与社会支持总分呈负相关(r=-0.208,P<0.01),社会支持总分与决策冲突总分呈负相关(r=-0.677,P<0.01)。决策冲突在晚期癌症患者代理决策者社会支持与决策疲劳间起部分中介作用,中介效应占47.5%。结论决策冲突是晚期癌症患者代理决策者社会支持与决策疲劳间的中介变量。护理管理者在对晚期癌症患者代理决策者进行干预时,不仅要注重提供直接的社会支持以减轻决策疲劳,还应通过开展针对性的咨询服务调节和化解决策冲突,帮助代理决策者降低决策疲劳感。
Objective To explore the mediating effect of decisional conflict between social support and decisional fatigue among proxy decision-makers in patients with advanced cancer.Methods Using convenience sampling,328 proxy decision-makers of advanced cancer patients receiving treatment at one affiliated cancer hospital of certain medical university from March to July 2024 were selected as research subjects.They were surveyed using the General Information Questionnaire,Decisional Fatigue Scale,Decisional Conflict Scale,and Social Support Assessment Scale.A structural equation model was constructed using AMOS 26.0 and the mediation effect model was tested using Bootstrap method.Results The total scores of decisional fatigue,decisional conflict and social support among proxy decision-makers in patients with advanced cancer in this group were 15.03±2.16,28.21±4.96,and 36.51±8.81 respectively.The total score of decisional fatigue was positively correlated with that of decisional conflict(r=0.330,P<0.01),while negatively with that of social support(r=-0.208,P<0.01),and that of decisional conflict(r=-0.677,P<0.01).Decisional conflict partially mediated the relationship between social support and decisional fatigue among proxy decision-makers in patients with advanced cancer,with a mediation effect accounting for 47.5%.Conclusion Decisional conflict is a mediating variable between social support and decisional fatigue among proxy decision-makers in patients with advanced cancer.It is recommended that nursing managers not only focus on providing direct social support to alleviate decisional fatigue when intervening with proxy decision-makers in patients with advanced cancer,but also provide targeted counseling services to regulate and resolve decisional conflicts,helping proxy decision-makers reduce decisional fatigue.
