基于术前MRI水肿区域的CT定位靶向骨水泥注射在老年骨质疏松性压缩骨折中的临床疗效研究

摘要 目的探讨基于术前MRI水肿区域的CT定位靶向骨水泥注射在老年骨质疏松性压缩骨折中的临床疗效。方法回顾性分析2022年10月至2024年6月期间在本院接受椎体后凸成形术的80例老年骨质疏松性压缩骨折患者。按照定位方式不同将其分为实验组和对照组,实验组40例,采用基于术前MRI水肿区域的CT定位靶向骨水泥注射方案,对照组40例,采用传统G型臂定位方案。比较两组患者的手术时间、住院时间、骨水泥渗漏率、术后疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)以及椎体前缘高度、椎体中部高度恢复情况。结果实验组手术时间、住院时间显著短于对照组(P<0.05);骨水泥渗漏率显著低于对照组(P<0.05)。实验组术后24 h、术后1个月VAS评分、ODI评分显著低于对照组(P<0.05)。实验组椎体前缘高度恢复率、椎体中部高度恢复率均显著高于对照组(P<0.05)。结论基于术前MRI水肿区域的CT定位靶向骨水泥注射技术能够有效缩短手术时间和住院时间,降低骨水泥渗漏率,改善疼痛及功能恢复情况,提高椎体高度恢复率,值得临床推广。 Objective To explore the clinical efficacy of CT targeted bone cement injection based on preoperative MRI edema area in elderly osteoporotic compression fractures.Methods 80 elderly patients with osteoporotic compression fractures who underwent kyphoplasty in our hospital from October 2022 to June 2024 were retrospective analysed.According to different positioning methods,they were divided into an experimental group and a control group.The experimental group of 40 cases used a CT targeted bone cement injection scheme based on preoperative MRI edema areas,while the control group of 40 cases used a traditional G-arm positioning scheme.The surgical time,hospitalization time,bone cement leakage rate,postoperative pain level[Visual Analogue Scale(VAS),score],Oswestry Disability Index(ODI),and recovery of anterior and middle vertebral body height between two groups of patients were compared.Results The surgical time and hospitalization time of the experimental group were significantly shorter than those of the control group(P<0.05);The leakage rate of bone cement was significantly lower than that of the control group(P<0.05).The VAS score and ODI score of the experimental group were significantly lower than those of the control group at 24 hours and 1 month after surgery(P<0.05).The recovery rates of anterior vertebral body height and middle vertebral body height in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion The CT targeted bone cement injection technology based on preoperative MRI edema areas can effectively shorten surgical time and hospitalization time,reduce bone cement leakage rate,improve patient pain relief and functional recovery,and increase vertebral height recovery rate,which is worthy of clinical promotion.
