摘要
目的比较经皮椎体成形术(PVP)中不同进针深度对骨水泥弥散及疗效的影响。方法按照穿刺针进针深度不同将62例行PVP治疗的骨质疏松性椎体压缩骨折(OVCF)患者分为A组(进针深度达全椎体前2/3,31例)和B组(进针深度达全椎体1/2,31例)。比较两组术中透视时间、术中透视次数、手术时间、术中骨水泥渗漏率、骨水泥分布满意率。采用ODI评分、疼痛VAS评分评价疗效。结果患者均获得随访,时间6~12个月。术中透视时间、术中透视次数、手术时间A组均短(少)于B组(P<0.05)。术中骨水泥渗漏率两组比较差异无统计学意义(P=0.065)。骨水泥分布满意率A组高于B组(P=0.001)。术后24 h疼痛VAS评分、ODI评分A组均优于B组(P<0.05)。结论与进针深度达全椎体1/2相比,进针深度达全椎体前2/3的PVP治疗OVCF可明显改善骨水泥弥散情况,减轻患者疼痛和改善腰椎功能。
Objective To compare the effects of different needle insertion depths on bone cement dispersion and therapeutic efficacy in percutaneous vertebroplasty(PVP).Methods The 62 patients with osteoporotic vertebral compression fractures(OVCFs)treated with PVP were divided into Group A(injection depth reaching anterior 2/3 of the whole vertebra,31 cases)and Group B(injection depth reaching 1/2 of the whole vertebra,31 cases)based on different puncture needle insertion depths.The intraoperative fluoroscopy time,intraoperative fluoroscopy frequency,surgical time,intraoperative bone cement leakage rate,and satisfaction rate of bone cement distribution were compared between two groups.The therapeutic effect was evaluated using ODI score and pain VAS.Results All patients were followed up for 6~12 months.The intraoperative fluoroscopy time,intraoperative fluoroscopy frequency,and surgical time in Group A were all shorter(less)than those in Group B(P<0.05).There was no statistical difference in the intraoperative bone cement leakage rate between the two groups(P=0.065).The satisfaction rate of bone cement distribution in Group A was higher than that in Group B(P=0.001).The pain VAS and ODI score of postoperative 24 h after surgery were better in Group A than those in Group B(P<0.05).Conclusions Compared with a needle insertion depth of 1/2 of the whole vertebra,PVP surgery with a needle insertion depth of 2/3 of the anterior vertebral body can statistically improve the diffusion of bone cement,alleviate patient pain,and improve lumbar spine function in the treatment of OVCF.
