摘要
在临床诊治脱发的过程中,孙占学教授发现中医“广义玄府”这一概念与毛囊这一解剖结构有吻合之处,可从“玄府失司”角度对脱发的病机加以阐释,提出其治疗以“调畅玄府,通达气血”为原则。虚而玄府失养者,治以“益气补血,升阳固卫,补虚和玄”,自拟养玄荣发汤;实而玄府郁闭者,治以“清热利湿,升举疏散,解郁通玄”,自拟通玄生发汤。另外,根据患者临床特点及证候表现,灵活调整用药。并重视中医外治法的应用,予患者自拟生发酊、消脂固发洗发颗粒外用,配合中药内服,临床取得良好疗效。
In the clinical diagnosis and treatment of Alopecia,Professor Sun Zhanxue noticed that the concept of Xuanfu in traditional Chinese medicine(TCM),in a broad sense,aligns with the anatomical structure of hair follicles.He proposed that the pathogenesis of Alopecia can be explained from the perspective of disorder of Xuanfu and established the treatment principle of regulating Xuanfu and promoting the flow of Qi and blood.For cases of deficiency with Xuanfu losing nourishment,the treatment involves tonifying Qi and blood,raising Yang and consolidating defense aspect,nourishing deficiency and harmonizing Xuanfu,and a self-formulated formula Yangxuan Rongfa decoction is applied.For cases of excess with Xuanfu obstruction,the treatment involves clearing heat and draining dampness,raising and dispersing,relieving stagnation and unblocking Xuanfu,and a self-formulated Tongxuan Shengfa decoction is used.The formulas are adjusted based on the clinical characteristics and symptoms of individual patients.Additionally,he places great importance on external TCM therapies,and has formulated Shengfa tincture and Xiaozhi Gufa shampoo granules for external use.This comprehensive approach has achieved favorable clinical outcomes.
