摘要
目的通过对出口梗阻型便秘患者行针刺八髎穴治疗,探究针刺八髎穴对盆底肌及肛管直肠压力的影响。方法将纳入的60例出口梗阻型便秘患者用随机数字法分为两组,基础治疗给予小麦纤维素口服,对照组予电子生物反馈治疗,观察组予针刺八髎穴,两组治疗前、治疗14天后分别进行症状评分、肛管直肠压力测定及盆底肌电图检查,统计分析,比较两组患者治疗后便秘改善情况及与盆底肌的关系。结果两组治疗前后在排便时间、排便不尽感、排便费力及肛门坠胀感症状中差异有统计学意义(P<0.05),观察组疗效明显优于对照组。在肛管直肠压力方面,能明显改善肛管静息压、肛管最大收缩压、感觉容量阈值及持续感觉容量,差异有统计学意义(P<0.05)。对盆底肌方面均有改善,差异无统计学意义(P>0.05)。对照组总有效率83%(25/30),观察组总有效率为90%(27/30)。结论针刺八髎穴通过改善盆底肌敏感性、协调性及稳定性来改善出口梗阻型便秘患者的便不尽感、排便费力、排便时间及肛门坠胀感。
Objective To explore the effect of acupuncture at the Baliao points on pelvic floor muscles and anorectal pressure in patients with outlet obstructive constipation.Methods Sixty patients with outlet obstructive constipation were randomly divided into two groups by random number method.The basic treatment was given with wheat fiber orally.The control group was treated with electronic biofeedback therapy,while the observation group was treated with acupuncture at the Baliao points.Symptom scores,anorectal pressure measurement and pelvic floor electromyography were performed before treatment and 14 days after treatment in both groups.Statistical analysis was conducted to compare the improvement of constipation and the relationship with pelvic floor muscles in the two groups.Results There were statistically significant differences in defecation time,incomplete defecation sensation,defecation effort and anal distension sensation before and after treatment in both groups(P<0.05),and the therapeutic effect of the observation group was significantly better than that of the control group.In terms of anorectal pressure,it could significantly improve the resting pressure of the anal canal,the maximum contraction pressure of the anal canal,the sensory capacity threshold and the sustained sensory capacity,with statistically significant differences(P<0.05).Both groups had improvements in pelvic floor muscles,but the differences were not statistically significant(P>0.05).The total effective rate of the control group was 83%(25/30),and that of the observation group was 90%(27/30).Conclusion Acupuncture at the Baliao points can improve the incomplete defecation sensation,defecation effort,defecation time and anal distension sensation in patients with outlet obstructive constipation by improving the sensitivity,coordination and stability of the pelvic floor muscles.
