粪便SDC2基因甲基化检测在结直肠癌早期筛查中的价值

摘要 目的研究人粪便SDC2基因甲基化检测在结直肠癌(CRC)早期筛查中的临床价值。方法回顾性搜集2023年1月—2024年10月在消化科就诊的,具有肠道相关症状,并进行结肠镜、病理和粪便SDC2基因甲基化检测的300例患者的相关资料。以结肠镜检查或术后病理诊断作为金标准,使用ROC曲线及AUC评估筛查结果,分析SDC2基因甲基化检测对筛查CRC的敏感度、特异度、阳性预测值以及阴性预测值。结果300例患者中,CRC 16例、进展型腺瘤5例、结肠息肉15例、未见明显异常264例;患者中SDC2基因甲基化阳性者32例,阳性率为11%。粪便SDC2基因甲基化检测对CRC的敏感度为63%(10/16)、特异度92%(262/284)、阳性预测值为31%(10/32)、阴性预测值98%(262/268);AUC值为0.726(P=0.027)。结论患者粪便SDC2基因甲基化检测,对于CRC早期筛查具有较高的临床价值,同时该检测标本采集方便、无创、患者依从性好,临床上可以将其应用于CRC的早期筛查。 Objective To investigate the clinical value of human fecal SDC2 gene methylation detection in the early screening of colorectal cancer(CRC).Methods The relevant data of 300 patients who attended the department of gastroenterology with intestinal-related symptoms and underwent colonoscopy,pathology,and fecal SDC2 gene methylation detection from January 2023 to October 2024 were retrospectively collected.Using colonoscopy or postoperative pathological diagnosis as the gold standard,the ROC curve and AUC were used to evaluate the screening results,and the sensitivity,specificity,positive predictive value,and negative predictive value of SDC2 gene methylation detection for CRC screening were analyzed.Results Among the 300 patients,there were 16 cases of CRC,5 cases of advanced adenoma,15 cases of colonic polyps,and 264 cases with no obvious abnormalities.A total of 32 patients were positive for SDC2 gene methylation,with a positive rate of 11%.The sensitivity,specificity,PPV,and NPV of fecal SDC2 gene methylation detection for CRC were 63%(10/16),92%(262/284),31%(10/32),and 98%(262/268),respectively;the AUC value was 0.726(P=0.027).Conclusion Fecal SDC2 gene methylation detection in patients has high clinical value for early CRC screening.Meanwhile,this detection method features convenient specimen collection,non-invasiveness,and good patient compliance,so it can be applied in clinical practice for early CRC screening.
