基于双理论框架的肿瘤专科护士培训迁移困境质性研究

Qualitative study on training transfer challenges among oncology specialist nurse:a dual theoretical framework approach

摘要 目的基于培训迁移及转化学习理论,深入探讨肿瘤专科护士培训迁移困境根源,提出干预策略,为优化护理人才培养体系提供理论依据。方法采用描述性质性研究方法,对天津市某三级甲等医院的17名肿瘤专科护士及8名护理教学管理者进行半结构式访谈,运用Colaizzi 7步分析法分析资料。结果肿瘤专科护士培训迁移困境包括认知-能力转化障碍(培训与实践脱节致知识重构困难);能力-资源动态失衡(需求超载与三重支持失能);评价-文化协同抑制(发展性评估断裂与反思生态抑制);文化-认知恶性循环(经验依赖固化与循证转化阻滞)。结论应构建“认知转化-能力建构-评价革新-资源协同-文化重塑”五环干预策略,以批判性反思与情境化培训为核心,以改善患者结局为目标,通过动态反馈驱动的评价体系,结合跨学科协作与循证实践整合,协同构建支持性组织环境与创新激励机制,推动肿瘤专科护士从“知识接受”向“实践转化”的转型。 Objective To explore the dilemmas in training transfer among oncology specialist nurses and put forward five-loop intervention strategies based on training transfer theory and transformative learning theory,aiming to provide theoretical insights for optimizing nursing talent development systems.Methods A descriptive research design was adopted.Semi-structured interviews were conducted with 17 oncology specialist nurses and 8 nursing education managers from a grade-A tertiary hospital in Tianjin.The data were analyzed using Colaizzi′s seven-step analysis method.Results The training was faced with four dimension-collaborative dilemmas:cognitive-ability transformation dilemma(the gap between training and practical application results in challenges for knowledge reconstruction),dynamic imbalance between capability and resources(demand overload and triple support failure),evaluation-culture inhibition(experience dependence solidification and evidence-based transformation obstruction)and cultural-cognition vicious cycle(developmental assessment rupture and reflective ecological inhibition).Conclusion The five loop intervention strategy of“cognitive transformation-ability construction-evaluation innovation-resource collaboration-cultural reshaping”should be constructed,with critical reflection and situational training as the core,aiming to improve patients′outcomes through a dynamic feedback-driven evaluation system.It integrates interdisciplinary collaboration and evidence-based practice,while simultaneously fostering a supportive organizational environment and innovative incentive mechanisms.The ultimate goal is to facilitate the transition of oncology specialty nurses from passive knowledge acquisition to active practice transformation.
