摘要
目的 比较妊娠期糖尿病(GDM)孕妇血糖、血脂水平变化和妊娠不良结局。方法 选取2023年1月至2025年1月在岳阳市人民医院就诊的82例妊娠糖尿病孕妇作为观察组,选取同期正常分娩的孕妇82例作为对照组。使用全自动生化分析仪对两组孕妇的样本进行甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FBG)、口服葡萄糖耐量试验(OGTT) 1 h血糖(OGTT 1hPG)、OGTT 2 h血糖(OGTT 2hPG)、糖化血清白蛋白(GA)和糖化血红蛋白(HbA1c)检测,分析并比较两组孕妇的血糖、血脂代谢相关指标和妊娠不良结局。结果 观察组FBG、OGTT 1hPG、OGTT 2hPG、GA、HbA1c、TC、TG、LDL-C、TC/HDL-C、TG/HDL-C、LDL-C/HDL-C和FBG/HDL-C以及妊娠不良结局发生率均高于对照组(P<0.05),而HDL-C水平低于对照组(P<0.05)。结论 妊娠期糖尿病孕妇的血糖、血脂等生化指标均异常,且妊娠期糖尿病对胎儿的发育和妊娠结局产生负面影响。建议临床医生加强孕妇生化相关指标的检查,联合分析,早期识别代谢异常并采取相应的干预措施,改善妊娠不良结局。
【Objective】To compare the changes in blood glucose,lipid levels and adverse pregnancy outcomes of pregnant women with gestational diabetes mellitus(GDM).【Methods】A total of 82 pregnant women with gestational diabetes who visited Yueyang People's Hospital from January 2023 to January 2025 were selected as the case group.Eighty-two pregnant women who delivered normally during the same period were selected as the control group.A fully automated biochemical analyzer was used to test the samples from both groups of pregnant women for triglycerides(TG),total cholesterol(TC),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C),fasting blood glucose(FBG),1-hour blood glucose(1hPG),2-hour blood glucose(2hPG),glycated serum albumin(GA),and glycated hemoglobin A1c(HbA1c).The blood glucose,lipid metabolism-related indexes,and adverse pregnancy outcomes of the two groups of pregnant women were analyzed and compared.【Results】Compared with the control group,the levels of FBG,1hPG,2hPG,GA,HbA1c,TC,TG and LDL-C and the incidence of adverse pregnancy outcomes in the case group were significantly increased(P<0.05),while the levels of HDL-C were significantly decreased(P<0.05).【Conclusion】Biochemical indexes such as blood glucose and blood lipids of pregnant women with gestational diabetes are abnormal,and gestational diabetes has a negative impact on fetal development and pregnancy outcomes.It is recommended that clinicians enhance the examination of biochemical indexes related to pregnant women,combine analysis,identify metabolic abnormalities early and take appropriate intervention measures to improve the adverse pregnancy outcomes.
