摘要
目的:探讨外周血D二聚体(D-Dimer,D-D)和中性粒细胞/淋巴细胞比率(Neutrophilto 1ymphocyte rate,NLR)在2型糖尿病患者静脉血栓形成中的预测价值。方法:选取2022年10月至2024年5月于本院进行诊治的2型糖尿病伴静脉血栓患者52例纳入血栓组。同时选取同期于本院进行诊治的无静脉血栓的2型糖尿病患者52例纳入无血栓组。收集对比两组的基线资料;检测对比两组的外周血D-D和NLR水平。采用多因素Logistic回归分析,探讨D-D和NLR与静脉血栓形成的相关性。利用受试者工作特征(Receiver operating characteristic,ROC)曲线分析D-D和NLR在静脉血栓形成中的预测价值。结果:血栓组的糖尿病周围血管病变、未规范用药、长期卧床、手术史/创伤史比例均显著高于无血栓组(P<0.05)。血栓组的D-D、NLR水平均显著高于无血栓组(P<0.05)。Logistic回归分析结果显示,合并糖尿病周围血管病变、未规范用药、长期卧床、手术史/创伤史、D-D、NLR水平上调是2型糖尿病患者发生静脉血栓的危险因素(P<0.05)。ROC结果显示,D-D和NLR联合预测2型糖尿病患者发生静脉血栓的曲线下面积为0.865,灵敏度为86.51%,特异度为72.112%。结论:合并糖尿病周围血管病变、未规范用药、长期卧床、手术史/创伤史、D-D、NLR水平上调是2型糖尿病患者发生静脉血栓的危险因素。D-D和NLR在2型糖尿病患者静脉血栓形成的预测中具有临床价值。
Objective:To investigate the predictive value of D-Dimer(D-D)and Neutrophilto 1ymphocyte rate(NLR)in peripheral blood in patients with type 2 diabetes mellitus.Methods:Fifty-two patients with type 2 diabetes mellitus with venous thrombosis who were treated in our hospital from October 2022 to May 2024 were included in the thrombus group.At the same time,52 patients with type 2 diabetes without venous thrombosis who were treated in our hospital during the same period were included in the thrombus free group.Baseline data were collected and compared between the two groups.The D-D and NLR levels in peripheral blood of the two groups were detected and compared.Multivariate Logistic regression analysis was used to investigate the correlation between D-D and NLR and venous thrombosis.The predictive value of D-D and NLR in venous thrombosis was analyzed using Receiver operating characteristic(ROC)curves.Results:The proportion of diabetic peripheral vascular disease,unregulated medication,long-term bed rest,surgical history/trauma history in thrombus group was significantly higher than that in non-thrombus group(P<0.05).The levels of D-D and NLR in thrombus group were significantly higher than those in non-thrombus group(P<0.05).Logistic regression analysis showed that diabetic peripheral vascular disease,irregular medication,long-term bed rest,surgical history/trauma history,D-D and NLR upregulated levels were risk factors for venous thrombosis in type 2 diabetes patients(P<0.05).ROC results showed that the area under the curve of D-D and NLR combined predicted the occurrence of venous thrombosis in type 2 diabetes patients was 0.865,the sensitivity was 86.51%,and the specificity was 72.112%.Conclusion:Diabetic peripheral vascular disease,unregulated medication,long-term bed rest,surgical history/trauma history,D-D and NLR up-regulated levels are risk factors for venous thrombosis in type 2 diabetes patients.D-D and NLR have clinical value in the prediction of venous thrombosis in patients with type 2 diabetes.
