摘要
目的探讨中性粒细胞计数、D-二聚体(D-D)指标与神经外科术后患者发生深静脉血栓(DVT)的相关性。方法本研究选择116例2021年1月至2024年1月于济源市中医院接受神经外科手术的患者,对神经外科术后发生DVT情况进行回顾性分析,对发生组和未发生组患者凝血相关指标、血常规相关指标水平进行检测并对比,采用多因素logistic回归分析筛选神经外科术后患者发生DVT的影响因素。结果神经外科术后发生出现DVT 25例(DVT组)、未发生DVT 91例(非DVT组);DVT组患者中性粒细胞计数指标水平比非DVT组升高,而血红蛋白指标水平则比非DVT组低(t=5.204、-2.254,均P<0.05);DVT组患者D-D、纤维蛋白原(FIB)指标水平均比非DVT组高(均P<0.05);多因素logistic回归分析结果得出,术前中性粒细胞计数[OR值(95%CI)=6.653(3.623,12.214),P<0.05]、D-D[OR值(95%CI)=1.131(1.027,1.245),P<0.05]指标水平上升为神经外科术后患者发生DVT的危险因素。结论中性粒细胞计数、D-D指标与神经外科术后患者发生DVT密切相关。
Objective To explore the relationship between neutrophil count,D-dimer(D-D)index and the occurrence of deep vein thrombosis(DVT)in patients after neurosurgery.Methods In this study,116 patients who received neurosurgery in Jiyuan Traditional Chinese Medicine Hospital from January 2021 to January 2024 were selected for retrospective analysis.The occurrence of DVT after neurosurgery was analyzed,and the levels of coagulation related indexes and blood routine related indexes of patients in the occurrence group and the non-occurrence group were detected and compared.Multivariate logistic regression analysis was used to screen the influencing factors of postoperative DVT.Results There were 25 cases in DVT group and 91 cases in non-DVT group after neurosurgery.The level of neutrophil count index in DVT group was higher than that in non-DVT group,while the level of hemoglobin index was lower than that in non-DVT group(t=5.204,-2.254,all P<0.05).The D-D and FIB indexes in DVT group were higher than those in non-DVT group(all P<0.05).Multivariate logistic regression analysis showed that preoperative neutrophil count[OR(95%CI)=6.653(3.623,12.214),P<0.05]and D-D index[OR(95%CI)=1.131(1.027,1.245),P<0.05]level increased as risk factors for DVT after neuro-surgery.Conclusion Neutrophil count and D-D index are closely related to the occurrence of DVT after neurosurgery.
