摘要
目的了解脑胶质瘤患者经历多次手术后的经济毒性体验及应对方式,为制定针对性医疗支持策略提供参考依据。方法采用目的抽样法,于2023年11月至2024年4月选取四川省某三甲医院神经外科27例脑胶质瘤患者进行半结构式访谈,应用Giorgi现象学分析法整理资料并提炼主题。结果经济毒性体验提炼出4个主题:直接医疗费用和间接损失造成经济毒性,心理压力大、无价值感和社会参与障碍,医疗决策困境以及生活质量下降;应对方式提炼出4个主题:开源节流、弹性工作,获得家庭支持、寻求社会援助,自我心理调适以及提出应对需求。结论医护人员应重视脑胶质瘤多次手术后患者的经济毒性,充分与其沟通成本效益,帮助患者获得家庭和社会支持,减轻经济毒性的负面影响。
Objective To understand the financial toxicity experiences and coping strategies in glioma patients following multiple surgical resections,so as to provide reference for formulating targeted medical support strategies.Methods A purposeful sampling method was employed between November 2023 and April 2024 to recruit 27 glioma patients in the Department of Neurosurgery at a tertiary grade A hospital in Sichuan Province for semi-structured interviews.And the data were analyzed and themes extracted using Giorgi's phenomenological analysis approach.Results Four themes emerged regarding financial toxicity experiences:financial toxicity due to direct medical costs and indirect financial losses;psychological distress,social participation barriers,and diminished self-worth;medical decision-making dilemmas;and reduced quality of life.Four coping strategy themes were identified:income diversification and expense reduction through flexible work arrangements;accessing family support and seeking social assistance;self-psychological adjustment;and articulating unmet support needs.Conclusion Health care professionals should pay attention to the financial toxicity in glioma patients following multiple surgical resections,fully communicate with them about the cost-benefits,and help them get family and social support,so as to mitigate the negative impact of financial toxicity.
