摘要
目的 探讨视网膜光损伤患者血清血管内皮生长因子(VEGF)及一氧化氮合酶(NOS)水平的变化规律,及其对视网膜光损伤的诊断效能。方法 纳入2022年1月至2024年12月确诊的视网膜光损伤患者168例(光损伤组),根据眼底病变程度分为轻度(56例)、中度(56例)、重度(56例)亚组,另选40例健康体检者为对照组。采用酶联免疫吸附测定法检测血清VEGF水平,化学比色法检测血清NOS水平。利用Pearson相关性分析VEGF与NOS水平的相关性。通过受试者工作特征曲线(ROC)分析VEGF与NOS对视网膜光损伤的诊断效能。结果 视网膜光损伤患者血清VEGF、NOS水平显著高于正常对照组,且随着视网膜光损伤程度的加重,患者血清VEGF、NOS水平逐渐升高,差异具有统计学意义(P<0.001)。Pearson相关性分析结果显示,VEGF与NOS呈正相关(r=0.765,P<0.001)。VEGF的AUC为0.912,灵敏度86.70%、特异度92.50%;NOS的AUC为0.843,灵敏度78.30%、特异度80.00%。VEGF与NOS对视网膜光损伤均具有较高的诊断价值,VEGF的诊断准确性优于NOS(Youden指数:0.792 vs. 0.583)。结论 视网膜光损伤患者血清VEGF及NOS水平显著升高,且与疾病严重程度呈正相关。VEGF与NOS水平对视网膜光损伤均具有较高的诊断价值,其中VEGF的诊断效能更优,可作为潜在的生物学标志物用于临床辅助诊断及病情评估。
Objective To investigate the changes in serum levels of vascular endothelial growth factor(VEGF)and nitric oxide synthase(NOS)in patients with retinal light injury and their diagnostic efficacy for the condition.Methods A total of 168 patients diagnosed with retinal light injury from January 2022 to December 2024 were included in this study(light injury group).Patients were subdivided based on the severity of retinal lesions into mild(56 cases),moderate(56 cases),and severe(56 cases)subgroups.Additionally,40 healthy individuals served as the control group.Serum VEGF levels were measured using enzyme-linked immunosorbent assay(ELISA),while serum NOS levels were assessed using colorimetric methods.Pearson correlation analysis was employed to evaluate the correlation between VEGF and NOS levels.The diagnostic efficacy of VEGF and NOS for retinal light injury was analyzed using receiver operating characteristic(ROC)curves.Results Serum levels of VEGF and NOS in patients with retinal light injury were significantly higher than those in the normal control group,with levels increasing progressively with the severity of retinal light injury(P<0.001).Pearson correlation analysis indicated a positive correlation between VEGF and NOS(r=0.765,P<0.001).The area under the curve(AUC)for VEGF was 0.912,with a sensitivity of 86.70%and specificity of 92.50%.The AUC for NOS was 0.843,with a sensitivity of 78.30%and specificity of 80.00%.Both VEGF and NOS demonstrated high diagnostic value for retinal light injury,with VEGF exhibiting superior diagnostic accuracy compared to NOS(Youden index:0.792 vs.0.583).Conclusion Serum levels of VEGF and NOS are significantly elevated in patients with retinal light injury and are positively correlated with disease severity.Both markers possess high diagnostic value for retinal light injury,with VEGF showing superior diagnostic efficacy.VEGF may serve as a potential biological marker for clinical auxiliary diagnosis and disease assessment.
