摘要
目的:探讨视网膜静脉阻塞继发黄斑水肿患者实施玻璃体腔注射雷珠单抗联合激光光凝术治疗的临床效果。方法:将北京市昌平区中西医结合医院眼科收治的100例视网膜静脉阻塞继发黄斑水肿患者(收治时间2024年2月—2025年2月)根据随机数字表法分为对照组和观察组,各50例。对照组采用激光光凝术治疗,观察组采用玻璃体腔注射雷珠单抗联合激光光凝术治疗。对比两组黄斑中心凹无血管面积(FAZ)、中央凹下脉络膜厚度(SFCT)、最佳矫正视力(BCVA)。结果:术前,两组FAZ对比,无统计学差异(P>0.05);术后1个月,两组FAZ减小(P<0.05),且观察组小于对照组(P<0.001)。术前,两组SFCT、BCVA对比,无明显差异(P>0.05);术后1个月,两组SFCT均降低,与对照组相比,观察组更低;两组BCVA提高,且观察组高于对照组(P<0.05)。结论:视网膜静脉阻塞继发黄斑水肿患者实施玻璃体腔注射雷珠单抗联合激光光凝术治疗的临床效果显著,可改善患者视力,减轻水肿程度。
Objective:Exploring the improvement of visual acuity in patients with macular edema secondary to retinal vein occlusion after combined treatment with intravitreal injection of ranibizumab and laser photocoagulation.Methods:A total of 100 patients with macular edema secondary to retinal vein occlusion admitted to the Ophthalmology Department of Beijing Changping Integrated Traditional Chinese and Western Medicine Hospital(February 2024 to February 2025)were randomly assigned using a random number table to either a control group or an observation group,comprising 50 patients each.The control group received laser photocoagulation therapy,while the observation group received intravitreal ranibizumab injection combined with laser photocoagulation therapy.The following parameters were compared between groups:foveal avascular zone(FAZ)area,subfoveal choroidal thickness(SFCT),and best-corrected visual acuity(BCVA).Results:Preoperatively,no statistically significant difference was observed in FAZ area between groups(P>0.05).At 1 month postoperatively,FAZ decreased in both groups(P<0.05),with the observation group showing a smaller reduction than the control group(P<0.001).Preoperatively,no significant differences were observed in SFCT or BCVA between the two groups(P>0.05).At 1 month postoperatively,both SFCT and BCVA decreased in both groups,with the observation group exhibiting lower values compared to the control group(P<0.05).Conclusion:Combining intravitreal ranibizumab injection with laser photocoagulation in patients with macular edema secondary to retinal vein occlusion yields significant clinical efficacy,improving visual acuity and reducing edema severity.
