摘要
目的通过系统评价和试验序贯分析(TSA)评估补肾活血汤治疗腰椎间盘突出症的治疗效果及安全性。方法计算机检索2004年1月1日至2024年12月31日中国生物医学文献数据库(CBM)、中国知网(CNKI)、维普中文科技期刊数据库(VIP)、万方、PubMed、Medline和Web of Science数据库,主题为补肾活血汤治疗腰椎间盘突出症的临床疗效,对检索到的文献进行筛选和数据提取,对提取的结局指标数据使用RevMan5.4和TSA 0.9.5进行Meta分析和TSA分析。结果本研究共纳入文献14篇,患者1184例。Meta分析显示,与对照组相比,治疗组治疗腰椎间盘突出症总有效率较高[RR=1.14,95%CI(1.05,1.24),P=0.003],治疗组可显著降低腰椎疼痛视觉模拟量表评分[SMD=-0.96,95%CI(-1.41,-0.52),P<0.0001],显著改善日本骨科协会评分[SMD=1.27,95%CI(0.85,1.68),P<0.00001]和Oswestry功能障碍指数[SMD=-2.66,95%CI(-4.19,-1.14),P=0.0006],且在不良反应发生方面差异无统计学意义。结论补肾活血汤治疗腰椎间盘突出症疗效显著,安全性良好,且结论可排除假阳性。
Objective:To evaluate the efficacy and safety of Bushen Huoxue Decoction(BSHXD)in treating lum-bar disc herniation(LDH)through a systematic review and trial sequential analysis(TSA).Methods:Databases including CBM,CNKI,VIP,Wanfang,PubMed,Medline,and Web of Science were electronically searched from Jan-uary 1,2004,to December 31,2024,for studies on the clinical efficacy of BSHXD in treating LDH.Retrieved liter-ature was screened,and data were extracted.Outcome data were analyzed using RevMan 5.4 and TSA 0.9.5 for me-ta-analysis and TSA.Results:40 studies involving 1184 patients were included.Meta analysis showed that,com-pared with the control group,the BSHXD group had a significantly higher total effective rate[RR=1.14,95%CI(1.05,1.24),P=0.003],significantly reduced visual analog scale(VAS)scores for lumbar pain[SMD=-0.96,95%CI(-1.41,-0.52),P<0.0001],and significantly improved Japanese Orthopaedic Association(JOA)scores[SMD=1.27,95%CI(0.85,1.68),P<0.00001]and Oswestry Disability Index(ODI)scores[SMD=-2.66,95%CI(-4.19,-1.14),P=0.0006].No significant difference was observed in the incidence of adverse events.Conclu-sion:BSHXD demonstrates significant efficacy and good safety in treating lumbar disc herniation,and the conclu-sion is robust against false-positive results.
