摘要
目的 观察内镜下腰椎椎间融合术与单纯腰椎减压术在治疗老年腰椎间盘突出症(LDH)患者的临床效果。方法 回顾性收集2023年10月至2025年3月期间于本院接收手术治疗的LDH患者104例,按手术方法不同分为对照组与观察组各52例,其中观察组行内镜下腰椎椎间融合术,对照组行内镜下单纯腰椎减压术。比较两组手术相关指标、术后腰椎疼痛、腰椎功能、椎管占位改善率、并发症情况。结果 观察组手术时间较对照组长(P<0.05);观察组术后椎管占位改善率较对照组高(P<0.05);两组术后1个月、3个月VAS评分、ODI评分降低(P<0.05),且观察组上述指标评分更低(P<0.05);对照组并发症发生率为5.77%,观察组为3.85%,两组差异无统计学意义(P>0.05)。结论内镜下腰椎椎间融合术可解除老年LDH患者神经根压迫,缓解腰腿疼痛效果更佳,术后腰椎稳定性更好,腰痛发生率低,且椎管容积改善率更好,利于术后腰椎功能的恢复,术后并发症少,安全性高,且复发率低。
Objective To compare the clinical efficacy of endoscopic lumbar interbody fusion(ELIF)and simple lumbar decompression in elderly patients with lumbar disc herniation(LDH).Methods A retrospective analysis was conducted on 104 LDH patients who underwent surgery in our hospital from October 2023 to March 2025.Patients were divided into two groups based on surgical approach:the control group(n=52,treated with endoscopic simple lumbar decompression)and the observation group(n=52,treated with ELIF).Surgical indicators,postoperative lumbar pain,lumbar function,spinal canal occupancy improvement rate,and complications were compared between the two groups.Results The operation time was longer in the observation group compared to the control group(P<0.05).The observation group showed a higher spinal canal occupancy improvement rate postoperatively(P<0.05).Both groups exhibited reduced Visual analog scale(VAS)and Oswestry disability index(ODI)scores at 1 and 3 months postoperatively(P<0.05),with lower scores in the observation group(P<0.05).The complication rate was 5.77%in the control group and 3.85%in the observation group,with no statistically significant difference(P>0.05).Conclusion ELIF effectively relieves nerve root compression in elderly LDH patients,provides better pain relief,enhances postoperative lumbar stability,reduces the incidence of low back pain,improves spinal canal volume,promotes functional recovery,and demonstrates fewer complications and lower recurrence rates,ensuring high safety.
