摘要
目的探析诱发ICU患者肠内营养相关性腹泻的常见高危因素,并据此作为制定护理方案的参考。方法以回顾性方式选取2023年4月-2025年3月间在本院ICU科室采取肠内营养支持干预,符合进组标准患者100例作为观察对象。依据是否出现腹泻症状,分为对照组(无腹泻情况)、观察组(出现腹泻)。对两组患者临床数据资料进行比较,分析影响ICU肠内营养支持患者发生腹泻的相关性因素,制定护理方案。结果在ICU肠内营养支持患者中,有25例患者发生相关性腹泻,发生率25.00%。单因素分析显示,两组患者在年龄、入住ICU时间、急性生理与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分、禁食时间、低蛋白血症、使用抗生素、使用抑酸剂、肠内营养制剂类型、营养液温度、营养液输注速度、营养液日用量方面比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,年龄≥60岁、入住ICU时间≥7d、APACHEⅡ评分≥15分、禁食时间≥3d、低蛋白血症、使用抗生素、使用抑酸剂、使用整蛋白型肠内营养制剂、营养液温度<37℃、营养液输注速度>50mL/h、营养液日用量>1000mL是ICU患者肠内营养相关性腹泻的独立危险因素(P<0.05)。结论ICU患者肠内营养相关性腹泻发生率较高,受多种因素影响,临床护理人员应加强对高危因素的评估与干预,采取针对性护理措施,以降低腹泻发生率,提高患者的治疗效果与预后。
Objective To explore the common high-risk factors that induce enteral nutrition-related diarrhea in ICU patients and use this as a data reference for formulating nursing plans.Methods A retrospective approach was conducted on 100 patients who were admitted to the ICU department of our hospital from April 2023 to March 2025 and received enteral nutrition support intervention,and met the inclusion criteria.According to whether diarrhea symptoms occurred or not,they were divided into the control group(without diarrhea)and the observation group(with diarrhea).The clinical data of the two groups of patients were compared to analyze the factors influencing the occurrence of related diarrhea in patients receiving enteral nutrition support in the ICU department,and a nursing plan was formulated.Results Among the patients receiving enteral nutrition support in the ICU,25 patients experienced related diarrhea,with an incidence rate of 25.00%.Univariate analysis showed that there were comparisons between the two groups of patients in terms of age,length of stay in the ICU,Acute Physiology and Chronic Health Evaluation II(APACHE II)score,fasting time,hypoproteinemia,use of antibiotics,use of acid suppressants,type of enteral nutrition preparation,temperature of nutrient solution,infusion rate of nutrient solution,and daily dosage of nutrient solution.The difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that Age≥60 years old and admitted to the ICU Time≥7days,APACHEⅡscore≥15 points,fasting time≥3 days,hypoproteinemia,use of antibiotics,use of acid suppressants,use of whole protein enteral nutrition preparations,nutrient solution temperature<37℃,nutrient solution infusion rate>50mL/h,and daily nutrient solution dosage>1000mL are independent risk factors for enteral nutrition-related diarrhea in ICU patients(P<0.05).Conclusion The incidence of enteral nutrition-related diarrhea in ICU patients is relatively high and is influenced by multiple factors.Clinical nursing staff should strengthen the assessment and intervention of high-risk factors and take targeted nursing measures to reduce the incidence of diarrhea and improve the treatment effect and prognosis of patients.
