摘要
目的观察超声引导下脉冲射频术治疗眼带状疱疹性神经痛的临床效果。方法选择112例眼带状疱疹性神经痛患者,随机分为脉冲射频组和神经阻滞组,各56例。脉冲射频组患者接受超声引导下眶上神经脉冲射频治疗,神经阻滞组患者接受超声引导下眶上神经阻滞治疗。记录两组患者治疗前及治疗后第1、4、8、12周的疼痛数字评价量表(NRS)、匹斯堡睡眠质量评分(PSQI)、普瑞巴林日用量及治疗后第12周总有效率。结果脉冲射频组患者治疗后第1、4、8、12周的NRS评分和PSQI评分均明显低于神经阻滞组(t分别=11.59、10.53、9.43、11.74;9.83、13.66、11.53、12.00,P均<0.05)。脉冲射频组患者治疗后第4、8、12周的普瑞巴林日用量明显低于神经阻滞组(t分别=3.89、7.71、6.29,P均<0.05)。治疗后第12周,脉冲射频组患者总有效率明显高于神经阻滞组,差异有统计学意义(χ^(2)=9.31,P<0.05)。结论超声引导下脉冲射频术治疗眼带状疱疹性神经痛疗效确切,可以降低患者普瑞巴林用量,改善睡眠质量。
Objective To observe the efficacy of ultrasound-guided pulsed radio frequency(PRF)for ophthalmic her⁃pes zoster neuralgia.Methods A total of 112 ophthalmic herpes zoster neuralgia patients were randomly divided into the PRF group and the nerve block(NB)group,56 patients in each group.The PRF group was given ultrasound-guided PRF on the supraorbital nerve,and the NB group was given ultrasound-guided nerve block on the supraorbital nerve.Nu⁃meric rating scale(NRS),pittsburgh sleep quality index(PSQI)scales,and daily doses of pregabalin were adapted to evaluate the effects before treatment and 1st week,4th week,8th week,12th week after treatment,and the total efficiency at 12th week after treatment were recorded in two groups.Results The NRS score and PSQI score of patients in PRF group at the 1st,4th,8th,and 12th weeks after treatment were significantly lower than those in NB group(t=11.59,10.53,9.43,11.74,9.83,13.66,11.53,12.00,P<0.05).The daily dosage of pregabarin in PRF group at the 4th,8th,and 12 th weeks after treatment was significantly lower than that in NB group(t=3.89,7.71,6.29,P<0.05).At the 12th week after treatment,the total effective rate of patients in PRF group was significantly higher than that in NB group,and the difference was statistically significant(χ^(2)=9.31,P<0.05).Conclusion Ultrasound-guided pulsed radiofrequency surgery has a definite therapeutic effect on ophthalmic herpes zoster neuralgia,reduces the dosage of pregabalin in pa⁃tients,and improves sleep quality.
