摘要
目的:系统分析老年髋部骨折病人跌倒恐惧的评估工具、影响因素以及干预策略,旨在为临床提供有效的跌倒恐惧管理方法。方法:基于Arksey和O'Malley的范围综述报告框架,检索PubMed、Embase、the Cochrane Library、Web of Science、中国知网、万方数据库、维普数据库、中国生物医学文献数据库自建库至2025年1月31日发表的相关文献,由2名研究者独立进行筛选、汇总及分析。结果:共纳入22篇文献,涵盖主题包括跌倒恐惧评估、影响因素分析、干预措施评价等。结论:跌倒恐惧是老年人髋部骨折后常见的心理问题,且其程度具有时间动态变化的特征,其评估工具种类繁多,虽具有普适性,但影响因素复杂,尚需开发特异性跌倒恐惧测评工具。老年髋部骨折病人跌倒恐惧的干预措施主要为认知疗法和功能训练,未来有待结合老年髋部骨折术后假体植入、本体感觉建立及步态平衡等领域进行深入探索。
Objective:To systematically analyze the assessment tools,influencing factors,and intervention strategies for fear of falling(FOF)in elderly patients with hip fracture,aiming to provide effective clinical management methods for FOF.Methods:Based on the scoping review framework by Arksey and O'Malley,relevant literature published up to January 31,2025,was retrieved from PubMed,Embase,the Cochrane Library,Web of Science,CNKI,Wanfang Database,VIP Database,and China Biology Medicine disc(CBM).Two researchers independently screened,summarized,and analyzed the literature.Results:A total of 22 articles were included,covering topics including FOF assessment,analysis of influencing factors,and evaluation of intervention strategies.Conclusions:Fear of falling is a common psychological problem in elderly patients after hip fracture,and its severity exhibits dynamic changes over time.Although there are numerous assessment tools for FOF with general applicability,the influencing factors are complex,and there is a need to develop specific FOF assessment tools.Interventions for FOF in elderly hip fracture patients primarily include cognitive therapy and functional training.Future research should combine in-depth exploration in areas such as prosthetic implantation,proprioception establishment,and gait balance rehabilitation post-hip fracture surgery in the elderly.
