摘要
目的评价汉化版跌倒效能量表（FES）的信效度,为其评价我国社区老年人跌倒风险的价值提供参考。方法对英文国际版FES（FES-I）进行翻译、文化调适,得到试验版FES。由20名老年人填写试验版FES,完善后得到汉化版FES。采用方便取样方法,于2014年3—6月在锦州市区发放问卷420份,有效回收400份。选择40名调查对象于2周后进行重测。结果量表Cronbach＇sα系数为0.921,Guttman分半信度系数为0.914,各条目得分组内相关系数为0.472~0.849,各条目得分与量表总分相关系数为0.541~0.835（P〈0.05）。重测信度系数为0.906。5名护理学专业教师对量表进行评分,各条目内容效度指数（CVI）为0.822~1.000,全体一致性CVI为0.834,平均CVI为0.845。采用主成分分析法,经方差正交旋转后得到2个公因子,特征值均〉1,累计方差贡献率为69.82%。验证性因子分析（CFA）显示,卡方/自由度比（CMIN/DF）为1.14,拟合优度指数（GFI）为0.916,调整的拟合优度指数（AGFI）为0.903,近似误差均方根（RMSEA）为0.035,规范拟合指数（NFI）为0.943,相对适配指数（RFI）为0.942,增值拟合指数（IFI）为0.905,非归准适配指数（TLI）为0.947,比较拟合指数（CFI）为0.955,标准化回归系数0.435~0.668。汉化版FES评价社区老年人跌倒风险价值的ROC曲线下面积为0.741〔95%CI（0.689,0.794）,P〈0.05〕。总分临界点为35分时,灵敏度为0.71,特异度为0.63。结论汉化版FES评价社区老年人跌倒风险具有良好的信效度,对评价我国社区老年人跌倒效能或自信心具有一定的推广价值。
Objective To evaluate the reliability and validity of Chinese version of Falls Efficacy Scale（ FES） and provide references for the risk assessment of the elderly falling down. Methods The English version of FES-I was translated into Chinese with an adaptation to Chinese culture,and a trial version of FES was made. The trial version FES-I was filled by20 elderly people, after which the trial version was improved and the ultimate Chinese version was obtained. Totally 420 questionnaires were sent out and 400 valid ones were retrieved using convenient sampling method in Jinzhou from March to June in2014. Of the 400 respondents,40 were given the same questionnaire again two weeks later. Results Cronbach＇s α coefficient and split-half reliability coefficient of Guttman were 0. 921 and 0. 914 respectively. Every item score of the intraclasscorrelation coefficient was 0. 472-0. 849. The correlation coefficient of every item and total scale score was 0. 541-0. 835（ P 0. 05）.Test-retest reliability coefficient was 0. 906. Scores were give by 5 nursing professional teachers on the scale. Content validity index of every item（ CVI） was 0. 822-1. 000. CVI of the whole consistency and the average were 0. 834 and 0. 845. Two common factors were obtained after variance orthogonal rotation by principal component analysis and the cumulating contribution rate of the two common factors whose feature value 1 was 69. 82%. The confirmatory factor analysis（ CFA） indicated that the values of CMIN / DF,GFI,AGFI,RMSEA,NFI,RFI,IFI,TLI,CFI and standardization regression coefficient were 1. 14,0. 916,0. 903,0. 035,0. 943,0. 942,0. 905,0. 947,0. 955 and 0. 435-0. 668 respectively. Area under curve of ROC was 0. 741 〔95% CI（ 0. 689,0. 794）,P 0. 05〕by Chinese version of FES assessing the risk of the aged falling down. The sensibility and specificity were 0. 71 and 0. 63 respectively when the total score critical point of the scale was 35. Conclusion Chinese version of FES has a good reliability and validity in the risk assessment of the elderly falling down. Therefore,it has certain promotional value in the risk assessment.
