寻常痤疮与循环炎症蛋白的因果关系:双向孟德尔随机化研究 被引量:1

Exploring Causal Correlations Between Acne Vulgaris and Circulating Inflammatory Proteins:Bi-Directional Mendelian Randomisation Study

摘要 目的通过双向孟德尔随机化分析探究寻常痤疮与91个循环炎症蛋白的因果关系。方法从GWAS数据库获取91个循环炎症蛋白与寻常痤疮的公开数据集,以随机效应逆方差加权法(inverse variance weighted,IVW)、MR-Egger回归、加权中位数法(weight median,WM)、简单模式(simple mode)、加权众数法(weighted mode)5种模型对其因果关系进行双向孟德尔随机化分析。结果91个循环炎症蛋白作为暴露时,白细胞介素-10受体亚基(interleukin-10 receptor subunit beta,IL-10RⅡ)水平(OR=1.42,95 CI:1.16~1.74,P=0.001)与寻常痤疮存在正相关的因果关系,而白血病抑制因子受体(leukemia inhibitory factor receptor,LIFR)水平(OR=0.69,95 CI:0.51~0.94,P=0.020)与寻常痤疮之间存在负相关的因果关系;当以寻常痤疮作为暴露时,提示寻常痤疮与胱抑素D(Cystatin D)(OR=0.97,95 CI:0.95~1.00,P=0.041)、肿瘤坏死因子配体超家族成员14(tumor necrosis factor ligand superfamily member 14,TNFSF14)水平(OR=1.03,95 CI:1.00~1.06,P=0.044)之间存在因果关系。结论IL-10RⅡ水平升高可增加寻常痤疮的发病风险,LIFR水平升高可降低其发病风险;而Cystatin D、TNFSF14水平可作为寻常痤疮的严重程度评判指标。 Objective To investigate the causal relationship between acne vulgaris and 91 circulating inflammatory proteins by bi-directional Mendelian randomisation study.Methods Publicly available datasets for 91 circulating inflammatory proteins and acne vulgaris were obtained from GWAS databases.A bidirectional Mendelian randomization(MR)analysis was performed to assess causal relationships using five models:inverse variance weighted(IVW)with random effects,MR-Egger regression,weighted median(WM),simple mode,and weighted mode.Results Analysis considering the 91 circulating inflammatory proteins as exposures revealed a positive causal link between acne vulgaris and interleukin-10 receptor subunit beta(IL-10RⅡ)levels(OR=1.42,95 CI:1.16-1.74,P=0.001),and a negative association between acne vulgaris and leukemia inhibitory factor receptor(LIFR)levels(OR=0.69,95 CI:0.51-0.94,P=0.020).Conversely,when acne vulgaris was treated as the exposure,a related causal relationship emerged between acne vulgaris and Cystatin D(OR=0.97,95 CI:0.95-1.00,P=0.041),as well as tumor necrosis factor ligand superfamily member 14(TNFSF14)levels(OR=1.03,95 CI:1.00-1.06,P=0.044).Conclusion Elevated IL-10RⅡ levels potentially heighten the risk of acne vulgaris,while the increased level of LIFR can reduce the risk of developing acne vulgaris.Cystatin D and TNFSF14 levels may serve as valuable indicators for assessing acne vulgaris severity.
