摘要
目的:探讨中枢神经系统白血病患者脑脊液中细胞因子水平的变化,分析这些参数在鉴别和诊断中枢神经系统白血病中的作用,给临床诊断提供理论依据。方法:选取2020年12月至2024年06月期间在本院接受治疗的22例中枢神经系统白血病患者,24例非中枢神经系统白血病患者作为对照组。结果:与对照组相比,中枢神经系统白血病组脑脊液白细胞介素-10(interleukin-10,IL-10)、白细胞介素-8(interleukin-8,IL-8)以及总蛋白(total protein,TP)的水平显著升高(P均<0.05),白细胞介素-17A(interleukin-17A,IL-17A)和氯离子(chloride ion,Cl-)的水平显著下降(P均<0.05)。通过进一步分析结果显示,中枢神经系统白血病患者脑脊液中细胞浓度越高,IL-10、IL-8以及TP检测数值越高,IL-17A和Cl-检测数值越低。脑脊液细胞因子IL-8、IL-10、IL-17A以及三者联合检测预测中枢神经系统白血病曲线下面积(area under the curve,AUC)分别为0.705、0.729、0.793、0.820。结论:在疑似中枢神经系统白血病患者的诊断过程中,结合测定脑脊液中IL-8、IL-10以及IL-17A的水平变化,能有助于中枢神经系统白血病的诊断,值得临床推广使用。
Objective:To investigate the changes of cytokines in cerebrospinal fluid(CSF)of patients with central nervous system leukemia(CNSL),analyze the role of these parameters in the identification and diagnosis of CNSL,and provide a theoretical basis for clinical diagnosis.Methods:A total of 22 patients with CNSL treated in our hospital from December 2020 to June 2024 were selected,and 24 patients with non-CNSL were selected as the control group.Results:Compared with the control group,the levels of interleukin-10(IL-10),interleukin-8(IL-8),and total protein(TP)in the cerebrospinal fluid of the CNSL group were significantly increased(all P<0.05),while the levels of interleukin-17A(IL-17A)and chloride ion(Cl-)were significantly decreased(all P<0.05).Further analysis showed that the higher the cell concentration in the cerebrospinal fluid of patients with CNSL,the higher the detection values of IL-10,IL-8,and TP,and the lower the detection values of IL-17A and Cl-.The area under the curve(AUC)of IL-8,IL-10,IL-17A,and their combined detection in predicting CNSL were 0.705,0.729,0.793,and 0.820,respectively.Conclusion:In the diagnosis of suspected patients with CNSL,the combined measurement of changes in the levels of IL-8,IL-10,and IL-17A in cerebrospinal fluid can help in the diagnosis of CNSL and is worthy of clinical promotion and application.
