摘要
目的 探讨维持性血液透析患者发生血管通路感染的影响因素。方法 收集2022年8月至2024年7月于广元市第一人民医院进行维持性血液透析的99例患者的临床资料,按照血管通路感染情况将其分为感染组(n=15)和非感染组(n=84),分析维持性血液透析患者发生血管通路感染的危险因素。结果 两组患者年龄、糖尿病比例、透析时间、透析过程中穿刺次数、血红蛋白(Hb)水平、血清白蛋白(Alb)水平比较,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,糖尿病史、透析过程中穿刺次数≥10次、透析时间≥3年、年龄≥60岁均是维持性血液透析患者发生血管通路感染的独立危险因素(P﹤0.05),而Hb、Alb水平均是其保护因素(P﹤0.05)。结论 糖尿病史、透析过程中穿刺次数≥10次、透析时间≥3年、年龄≥60岁均是维持性血液透析患者发生血管通路感染的独立危险因素,Hb、Alb水平均是其保护因素。临床医护人员应高度重视这些因素,针对不同风险因素采取相应的预防和干预措施,从而降低血管通路感染的发生率,改善患者的透析质量和预后生存情况。
Objective To explore the risk factor of vascular access infection in maintenance hemodialysis patients.Method The clinical data of 99 patients who underwent maintenance hemodialysis at the Guangyuan First People's Hospital of from August 2022 to July 2024 were collected.They were divided into infection group(n=15)and non-infection group(n=84)based on the status of vascular access infection.The risk factors for vascular access infection in maintenance hemodialysis patients were analyzed.Result There were statistically significant differences in age,proportion of diabetes,dialysis duration,number of punctures during dialysis,hemoglobin(Hb)level,and serum albumin(Alb)level between the two groups(P<0.05).The results of multivariate analysis showed that a history of diabetes,puncture frequency during dialysis≥10 times,dialysis duration≥3 years,and age≥60 years old were independent risk factors for vascular access infection in maintenance hemodialysis patients(P<0.05),while Hb and Alb levels were protective factors(P<0.05).Conclusion A history of diabetes,puncture frequency during dialysis≥10 times,dialysis time≥3 years,and age≥60 years old are all independent risk factors for vascular access infection in maintenance hemodialysis patients,Hb and Alb levels are protective factors.Clinical medical staff should attach great importance to these factors and take corresponding preventive and intervention measures for different risk factors to reduce the incidence of vascular access infection and improve the dialysis quality and prognosis of patients.
