摘要
目的:探究脾肾双补化瘀方联合地屈孕酮对脾肾两虚兼血瘀型早期先兆流产的治疗效果。方法:将200例符合诊断标准的脾肾两虚兼血瘀型早期先兆流产患者纳入研究,随机分为两组,每组各100例。对照组使用保胎灵联合地屈孕酮治疗3周,试验组则给予脾肾双补化瘀方配合地屈孕酮连续3周治疗。随后比较两组患者用药后在临床疗效、血清激素水平及中医证候(阴道流血、腹部疼痛、腰部酸痛)评分的变化。结果:试验组总有效率为95.00%,对照组总有效率为87.00%。试验组的临床治疗效果显著优于对照组(P<0.05),保胎结局更佳。此外,试验组患者治疗3周后人绒毛膜促性腺激素(β-HCG)、治疗1周及3周后孕酮(P)水平均显著高于对照组(P<0.05),治疗后中医各项证候评分均显著低于对照组(P<0.05)。结论:相较于保胎灵联合地屈孕酮治疗,脾肾双补化瘀方联合地屈孕酮治疗脾肾两虚兼血瘀型早期先兆流产更能提高患者β-HCG及P水平,改善患者阴道流血和腰腹部不适,临床疗效显著,保胎成功率更高。
Objective:To explore the therapeutic effect of Pishen Shuangbu Huayu prescription combined with dydrogesterone on early threatened abortion of spleen-kidney deficiency combined with blood stasis type.Methods:200 patients with spleen and kidney deficiency combined with blood stasis type of early threatened abortion were enrolled in the study and randomly assigned to the control group and the experimental group,each consisting of 100 patients.The control group was treated with Baotailing combined with dydrogesterone for 3 weeks,whereas the experimental group was treated with Pishen Shuangbu Huayu prescription combined with dydrogesterone for 3 weeks.The clinical efficacy,serum hormone level and TCM syndrome score(vaginal bleeding,abdominal pain,waist pain)were compared between the two groups after medication.Results:In the experimental group,the total effective rate was 95.00%;In comparison,the control group the total effective rate was 87.00%effective rate.The clinical outcomes in the experimental group significantly better than those in the control group(P<0.05),with superior pregnancy maintenance results.After 3 weeks of treatment,the experimental group showed significantly elevated human chorionic gonadotropin(β-HCG)levels,and at both 1 and 3 weeks,progesterone(P)levels were notably higher compared to the control group(P<0.05);Additionally,post-treatment TCM syndrome scores were substantially lower than those in the control group(P<0.05).Conclusion:Compared with Baotailing combined with dydrogesterone in treatment of early threatened abortion of spleen-kidney deficiency and blood stasis type,Pishen Shuangbu Huayu prescription combined with dydrogesterone can improve theβ-HCG and P levels of patients,vaginal bleeding and abdominal discomfort of patients,with significant clinical efficacy and a higher success rate of fetal preservation.
