摘要
目的探讨加味茵陈蒿汤外洗对新生儿胆汁淤积性黄疸(NCJ)的疗效。方法选取80例NCJ患儿,随机分为观察组(n=40,常规治疗+加味茵陈蒿汤外洗)和对照组(n=40,仅常规治疗)。比较两组黄疸消退时间、肝功能指标[谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、直接胆红素(DBIL)下降值]、临床疗效及不良事件发生情况。结果观察组黄疸消退时间以及TBIL、DBIL、ALT、AST下降值分别为(7.21±1.56)d、(45.32±8.75)μmol/L、(18.26±4.14)μmol/L、(28.53±6.31)U/L、(24.80±5.95)U/L,均显著优于对照组的(10.83±2.14)d、(32.17±7.28)μmol/L、(14.65±3.82)μmol/L、(22.76±5.94)U/L、(19.34±4.78)U/L(P<0.05)。观察组总有效率与对照组比较更高(92.50%vs.72.50%)(P<0.05)。两组不良事件发生率相当(P>0.05)。结论加味茵陈蒿汤外洗可加速NCJ患儿的黄疸消退,改善肝功能,安全有效。
Objective To explore the efficacy of external washing with Jiawei Yinchenhao decoction on improving neonatal cholestatic jaundice(NCJ).Methods 80 children with NCJ were selected and randomly divided into observation group(n=40,conventional treatment+external washing with modified Yinchenhao decoction)and control group(n=40,conventional treatment only).Comparison of jaundice regression time,liver function indexes[decrease of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),and direct bilirubin(DBIL)],clinical efficacy,and occurrence of adverse events between the two groups.Results The jaundice regression time and the decrease of TBIL,DBIL,ALT and AST in the observation group were(7.21±1.56)d,(45.32±8.75)μmol/L,(18.26±4.14)μmol/L,(28.53±6.31)U/L and(24.80±5.95)U/L,which were significantly better than those in the control group[(10.83±2.14)d,(32.17±7.28)μmol/L,(14.65±3.82)μmol/L,(22.76±5.94)U/L,and(19.34±4.78)U/L](P<0.05).The total effective rate of the observation group was higher than that of the control group(92.50%vs.72.50%,P<0.05).The incidence of adverse events in the two groups was comparable(P>0.05).Conclusion External washing with Jiawei Yinchenhao decoction can accelerate the regression of cholestatic jaundice and improve liver function of NCJ children,and is safe and effective.
