摘要
目的探讨血清转化生长因子-β1(TGF-β1)、内皮素-1(ET-1)水平与视神经脊髓炎谱系疾病(NMOSD)复发的相关性。方法选取2020年1月至2022年12月南京医科大学附属明基医院神经内科收治的95例初诊治NMOSD患者为NMOSD组,根据出院后1年内是否复发分为复发组(38例)和未复发组(57例),选取同期50例健康体检者为正常对照组(NC组)。检测血清TGF-β1、ET-1水平,分析其对NMOSD复发的预测价值。结果NMOSD组患者血清TGF-β1水平低于NC组,而血清ET-1水平高于NC组(P<0.05),随着脊髓受累节段数的增加及扩展残疾状态量表(EDSS)评分的升高,NMOSD患者血清TGF-β1水平逐渐降低,而血清ET-1水平呈升高趋势(P<0.05)。NMOSD患者血清TGF-β1水平与脊髓受累节段数、EDSS评分呈负相关,血清ET-1水平与脊髓受累节段数及EDSS评分呈正相关关系(P<0.05)。复发组患者血清TGF-β1水平低于未复发组,血清ET-1水平则高于未复发组(P<0.05)。血清TGF-β1与ET-1联合预测NMOSD复发的曲线下面积为0.903,高于TGF-β1、ET-1单一指标检测的0.822、0.784(P<0.05)。结论NMOSD患者血清TGF-β1、ET-1表达异常,其水平与脊髓受累节段数及躯体残疾程度有关,血清TGF-β1、ET-1可作为评估NMOSD复发风险的参考指标。
Objective To investigate the correlation between serum transforming growth factor-β1(TGF-β1)and endothelin-1(ET-1)levels and the recurrence of neuromyelitis optica spectrum diseases(NMOSD).Methods A total of 95 patients with NMOSD who were initially diagnosed and treated in the Department of Neurology,BenQ Medical Center,the Affiliated BenQ Hospital of Nanjing Medical University from January 2020 to December 2022 were selected as NMOSD group,and were divided into recurrence group(38 cases)and non-recurrence group(57 cases)according to whether they relapsed within 1 year after discharge,and 50 healthy subjects who underwent physical examination during the same period were selected as normal control group(NC group).The levels of TGF-β1 and ET-1 in serum were detected and their predictive value for NMOSD recurrence was analyzed.Results The serum TGF-β1 level in NMOSD group was lower than that in NC group,while the serum ET-1 level was higher than that in NC group(P<0.05).With the increase of spinal cord involved segments and the increase of EDSS score,the serum TGF-β1 level in NMOSD patients was gradually decreased,while the serum ET-1 level was increased(P<0.05).In NMOSD patients,the serum TGF-β1 level was negatively correlated with the number of spinal cord involved segments and EDSS score,while the serum ET-1 level was positively correlated with the number of spinal cord involved segments and EDSS score(P<0.05).The serum TGF-β1 level in the relapsed group was lower than that in the non-relapsed group,and the serum ET-1 level was higher than that in the non-relapsed group(P<0.05).The AUC of TGF-β1 combined with ET-1 to predict NMOSD recurrence was 0.903,which was higher than that of TGF-β1 and ET-1(0.822 and 0.784)(P<0.05).Conclusion The expression of TGF-β1 and ET-1 in serum of NMOSD patients is abnormal,and their levels are related to the number of spinal cord segments involved and the degree of physical disability.Serum TGF-β1 and ET-1 can be used as a reference index to evaluate the risk of recurrence of NMOSD.
