糖尿病视网膜病变基层防治的数字化应用进展

摘要 糖尿病视网膜病变(DR)是可预防的致盲性眼病。DR起病隐匿,致盲率高,患者面临着严重的视力威胁和经济负担,其早期发现、早期治疗尤其重要。近年来,我国的分级诊疗制度不断完善,基层医疗机构逐渐成为DR防治中的重要环节,这对基层医疗机构的DR防治水平提出了更高要求。数字医疗技术的发展,优化了医疗资源的配置,并给基层医疗机构DR防治提供了新的思路和方法,带来了重要的发展机遇。该文从数字健康服务平台、远程医疗、人工智能、物联网、大数据五个方面综述了国内外数字技术在DR防治中的研究现状,展示在数字化背景下DR患者诊疗方式的变革,探讨数字技术在基层医疗DR防治中的优势及局限性,为在基层医疗机构开展DR防治的数字化研究与实践提供借鉴。 Diabetic retinopathy(DR)is a preventable blindness/ophthalmopathy with an insidious onset and a high rate of blindness.Patients with DR are confronted with severe threats of visual loss and economic burden,and the early detection and treatment of DR are therefore particularly crucial.In recent years,the tiered healthcare system in China has undergone many improvements,and primary care institutions have gradually emerged as an essential part in the prevention and control of DR,which requires higher capability of such institutions in this area.The development of digital medicine optimized the allocation of care resources,and provides new ideas and approaches for the primary care of DR,thus bring important opportunities for primary care providers.This paper reviews the current research status of the application of digital health technologies in preventing and treating DR in China and elsewhere from five aspects:digital health service platforms,telemedicine,artificial intelligence,the Internet of Things,and big data.It presents the transformation of DR diagnosis and treatment approaches in the digital era,examines the advantages and limitations of digital technologies in the prevention and treatment of DR in primary care,and offers references for the research and practice of digital medicine in DR prevention and treatment in primary care.
