摘要
目的应用胎儿心脏定量分析技术(FHQ)对肺动脉瓣狭窄(PVS)胎儿双心室各个节段大小形态及收缩功能进行评估,探讨其变化特点及心脏定量分析技术的应用价值。方法回顾性分析2021年1月—2023年9月在合肥市妇幼保健院产前超声诊断并产后确诊为孤立性PVS的胎儿(PVS组,44例)的病历资料,选取同时期正常胎儿100例作为正常组,运用FHQ技术分析双心室的大小、形状和收缩功能,比较PVS组胎儿与正常组之间的差异。结果PVS组胎儿较正常组的心脏整体更呈球形,其右室大小未见明显改变,左室第13~24节段ED增宽(P<0.05);右室第11~15节段、左室第19~24节段SI小于正常组(P<0.05)。PVS组胎儿右室的FAC、GLS、RVFWS及全节段FS均较正常组下降(P<0.05),但BALFS未见明显差异;而左室的心功能与正常组间未见明显差异。结论FHQ技术可以定量评估PVS胎儿双心室大小形态及收缩功能的变化,其左室中间段及心尖段横径增宽,心尖部呈球形改变;右室中间段呈球形改变,右室的整体收缩功能、部分纵向收缩功能及全节段横向收缩功能均明显降低。
Objective To evaluate the biventricular size、morphology and systolic function of different segments in fetuses with pulmonary valve stenosis(PVS)by applying the technique of fetal heart quantification,and to explore the characteristics of its changes and the value of the application of the technique of fetal heart quantification.Methods A total of 44 fetuses with isolated PVS diagnosed prenatally and postnatal diagnosed in our hospital from January 2021 to September 2023 were retrospectively analyzed,and 100 were normal control fetuses,and the sizes、shapes and contractile functions of the biventricular were analyzed by using the FHQ,the differences between the PVS group and the normal group were compared.Results The hearts shape of the fetuses in the PVS group were more spherical than those in the normal group,their right ventricular size was not significantly altered,and the ED of the left ventricular segments 13-24 was widened(P<0.05);and the SI of the right ventricular segments 11-15 and the left ventricular segments 19-24 were smaller than those of the normal group(P<0.05).FAC,GLS,RVFWS,and whole-segment transverse FS of the right ventricle of the fetuses in the PVS group were decreased compared with that of the normal group(P<0.05),but no significant difference was seen in BALFS;However,cardiac function of the left ventricle was not significantly different from that of the normal group.Conclusion FHQ can quantitatively assess the changes of biventricular size、morphology and systolic function in fetuses with PVS.The transverse diameters of the left ventricular intermediate and apical segments were widened,and the apical portion of the ventricle showed a spherical change;The right ventricular intermediate segment showed a spherical change,and the global systolic function of the right ventricle,part of the longitudinal systolic function,and the whole segments of the transverse systolic function were all significantly reduced.
