摘要
目的探究医护一体化护理对重症肺炎患儿临床症状缓解时间、血气分析指标及并发症发生率的影响。方法选择2021年1月至2023年12月于华中科技大学同济医学院附属同济医院治疗的102例重症肺炎患儿为研究对象,将其以随机数字表法进行分组,各51例。对照组患儿给予基础护理,观察组给予基础护理+医护一体化护理,比较2组的干预效果。结果观察组患儿咳嗽、气喘、肺啰音和发热的缓解时间均早于对照组(P均<0.05)。出院时,2组的动脉血二氧化碳分压(PaCO_(2))均较护理前降低,动脉血氧分压(PaO_(2))较护理前增高,pH值较护理前增大(P均<0.05),且观察组的PaCO_(2)低于对照组,PaO_(2)高于对照组,pH值大于对照组(P均<0.05)。观察组患儿气胸、上消化道出血和肺不张的并发症合计发生率为1.96%(1/51),低于对照组的15.69%(8/51),且数据组间比较有统计学差异(P<0.05)。结论医护一体化护理能够促进重症肺炎患儿血气分析指标的恢复,降低并发症发生率,缩短患儿的临床症状缓解时间。
Objective To explore the impact of integrated medical-nursing care on the clinical symptom relief time,blood gas analysis indicators,and incidence of complications in children with severe pneumonia.Methods A total of 102 children with severe pneumonia treated at Tongji Hospital,Tongji Medical College of Huazhong University of Science and Technology from January 2021 to December 2023 were selected as the subjects of the study.They were randomly divided into two groups using a random number table method,with 51 cases in each group.The control group received basic nursing,while the observation group received basic nursing and integrated medical-nursing care.The intervention effects of the two groups were compared.Results The relief time of cough,asthma,pulmonary rales,and fever in the observation group was earlier than that in the control group(P<0.05).At discharge,the arterial partial pressure of carbon dioxide(PaCO_(2))in both groups was lower than that before nursing,and the arterial partial pressure of oxygen(PaO_(2))and pH value in both groups were higher than that before nursing(P<0.05).Additionally,the observation group shower lower PaCO_(2),higher PaO_(2),and higher pH value compared with the control group(P<0.05).The overall incidence of complications such as pneumothorax,upper gastrointestinal hemorrhage,and pulmonary atelectasis in the observation group was 1.96%(1/51),which was lower than the 15.69%(8/51)in the control group.There was a statistically significant difference between the data groups(P<0.05).Conclusion The implementation of integrated medical-nursing care can promote the recovery of blood gas analysis indicators in children with severe pneumonia,reduce the incidence of complications,and shorten the clinical symptom relief time.
