摘要
目的探究益气扶正解毒汤加减联合生脉注射液治疗脓毒症的疗效及作用机制探讨。方法按简单随机化分组法将2021年6月—2023年6月在河南省安阳市中医院重症医学科就诊的瘀毒内阻证脓毒症患者分为观察组和对照组,各40例。对照组接受常规西医治疗;观察组接受益气扶正解毒汤加减联合生脉注射液治疗。比较两组患者治疗1周后的临床疗效,以及比较治疗前和治疗1周后的中医证候积分、病情严重程度[急性生理与慢性健康(Acute Physiology and Chronic Health Evaluation APACHEⅡ)、序贯器官衰竭估计(SOFA)]、炎症水平和凝血功能指标。结果治疗1周后,观察组的总有效率显著高于对照组(P<0.05)。治疗1周后,两组患者的中医证候积分、APACHEⅡ、SOFA评分、血清降钙素原(procal citonin,PCT)、C反应蛋白(C-reactire protein,CRP)、白细胞计数(white blood cell count,WBC)、凝血酶原时间(prothrombin time,PT)和活化部分凝血活酶时间(activated partial thromboplastin time APTT)较治疗前显著降低,且观察组低于同期对照组(P均<0.05);两组患者的PLT较治疗前显著提高,且观察组高于同期对照组(P均<0.05)。结论益气扶正解毒汤加减联合生脉注射液治疗能够有效缓解瘀毒内阻证脓毒症患者的症状,显著改善患者炎症水平和凝血功能指标,具有较好的安全性。
Objective To explore the efficacy and role mechanism of modified Modified Yiqi Fuzheng Jiedu Decoction(益气扶正解毒汤加减)combined with Shengmai Injection(生脉注射液)in the treatment of sepsis.Methods According to the simple randomization grouping method,the patients with sepsis of stasis-toxin and internal obstruction syndrome in intensive care unit of Anyang Hospital of Traditional Chinese Medicine from June 2021 to June 2023 were divided into observation group(40 cases)and control group(40 cases).The control group received conventional western medicine treatment,while the observation group was treated with Modified Yiqi Fuzheng Jiedu Decoction combined with Shengmai Injection.The clinical efficacy after 1 week of treatment and TCM syndromes scores,disease severity[Acute Physiology and Chronic Health Evaluation(APACHEⅡ),Sequential Organ Failure Assessment(Sequential Organ Failure Assessment SOFA)],inflammation level and coagulation function indicators before treatment and after 1 week of treatment were compared.Results After 1 week of treatment,the total effective rate in the observation group was significantly higher(P<0.05).The TCM syndromes scores,APACHEⅡscore,SOFA score,procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC).prothrombin time(PT)and activated partial thromboplastin time(APTT)after 1 week of treatment were significantly declined in both groups than those before treatment,and the above indicators in the observation group were lower(all P<0.05).The platelet count(PLT)in the two groups was significantly enhanced compared with that before treatment,and the observation group had higher PT(all P<0.05).Conclusion Modified Yiqi Fuzheng Jiedu Decoction combined with Shengmai Injection can effectively alleviate the symptoms of patients with sepsis of stasis-toxin and internal obstruction syndrome,and significantly improve the inflammation level and coagulation function indicators of patients,and it has good safety.
