摘要
目的:总结李玉锋教授中医药治疗慢性萎缩性胃炎的用药规律及理论思想。方法:收集2023年1月-2024年4月就诊于辽宁中医药大学附属医院李玉锋教授门诊的慢性萎缩性胃炎患者资料,提取患者首诊处方方药,导入中医传承计算平台系统(V3.0)进行统计分析。结果:共收集到105例慢性萎缩性胃炎患者的病例资料,105首处方共涉及中药160味。用药频次≥30次的药物有21味。四气以寒、温居多;五味以苦味、辛味、甘味为主;归经多归脾、肝、肺、胃四经。出现频率≥38的药物组合15个,置信度≥0.8的组合20条,包含中药7味,并根据聚类分析总结出4个核心处方。结论:李玉锋教授论治慢性萎缩性胃炎强调其“脾虚气滞,湿热血瘀”的病机,用药肝脾同调、寒温相济、灵活化裁,为临床治疗慢性萎缩性胃炎提供诊疗思路。
Objective:To summarize Professor Li Yufeng s medication rules and theoretical thoughts in the treatment of chronic atrophic gastritis with traditional Chinese medicine(TCM).Methods:The data of patients with chronic atrophic gastritis who were treated in Li Yufeng Clinic of the Hospital Affiliated to Liaoning University of Traditional Chinese Medicine from January 2023 to April 2024 were gathered.To extract the TCM in these patients first prescription,and enter into the TCM Inheritance computing platform system(V3.0)for statistics and analysis.Results:Data were collected from 105 cases of patients with chronic atrophic gastritis.A total of 160 Chinese herbs were involved in 105 prescriptions.21 kinds of TCM were used more than 30 times.The frequency of warm and cold drugs is the highest.The five flaours were mainly bitter,pungent and sweet.Most of the channel tropism belongs to the spleen,liver,lung,and stomach.There were 15 drug combinations with frequency≥38,20 combinations with confidence≥0.8,including 7 kinds of Chinese medicine,and 4 core prescriptions were summarized according to cluster analysis.Conclusion:Professor Li Yufeng,when discussing the treatment of chronic atrophic gastritis,emphasizes its pathogenesis of“spleen deficiency and Qi stagnation,dampness-heat and blood stasis”.In medication,he adjusts the liver and spleen simultaneously,combines cold and warm properties appropriately,and makes flexible modifications,thus providing ideas for the diagnosis and treatment of chronic atrophic gastritis in clinical practice.
