摘要
目的观察半夏厚朴汤合射干麻黄汤加减治疗小儿肺炎支原体肺炎(Mycoplasma Pneumoniae Pneumonia,MPP)风寒闭肺型的临床疗效。方法选取2023年10-12月在长沙市中医医院儿科就诊的MPP风寒闭肺型患儿52例,采用随机数字表法分为治疗组和对照组,各26例。2组均给予雾化、止咳化痰对症治疗,对照组在此基础上予阿奇霉素颗粒治疗,治疗组在对照组治疗方法的基础上联合半夏厚朴汤合射干麻黄汤加减治疗。2组均治疗5 d后统计临床疗效,比较治疗前后中医证候积分变化以及临床症状消失时间。结果治疗组总有效率为96.15%(25/26),显著高于对照组的65.38%(17/26),2组比较,差异有统计学意义(P<0.05)。治疗前,2组咳嗽、咳痰、睡眠、发热、大便干结证候积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后,2组咳嗽、咳痰、睡眠证候积分均明显降低,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组下降更显著(P<0.05)。治疗组咳嗽、咳痰、发热、肺部啰音消失时间显著短于对照组,2组比较,差异有统计学意义(P<0.05)。结论半夏厚朴汤合射干麻黄汤加减治疗小儿MPP风寒闭肺型可显著降低中医证候积分,缩短咳嗽、咳痰、发热、肺部啰音的消失时间,值得临床推广应用。
Objective To observe the clinical efficacy of modified Banxia Houpu Tang(半夏厚朴汤)combined with Shegan Mahuang Tang(射干麻黄汤)in treating mycoplasma pneumoniae pneumonia(MPP)of the wind-cold blocking lung type.Methods A total of 52 pediatric patients with MPP of wind-cold blocking lung type treated at the Department of Pediatrics,Changsha Hospital of Traditional Chinese Medicine from October to December 2023 were selected and randomly divided into a treatment group and a control group,with 26 cases each.Both groups received symptomatic treatments,including nebulization,antitussives,and expectorants.The control group was additionally treated with azithromycin granules,while the treatment group received the modified Banxia Houpu Tang combined with Shegan Mahuang Tang on top of the control group's regimen.Clinical efficacy was evaluated after 5 days of treatment,comparing changes in TCM syndrome scores and the disappearance time of clinical symptoms before and after treatment.Results The total effective rate in the treatment group was 96.15%(25/26),significantly higher than the control group's 65.38%(17/26),with a statistically significant difference(P<0.05).Before treatment,there was no significant difference in syndrome scores(cough,expectoration,sleep,fever,stool)between the two groups(P>0.05).After treatment,both groups showed significant reductions in cough,expectoration,and sleep scores compared to pre-treatment(P<0.05),with a more pronounced decrease in the treatment group(P<0.05).The disappearance times of cough,expectoration,fever and lung rales in the treatment group were significantly shorter than those in the control group(P<0.05).Conclusion Modified Banxia Houpu Tang combined with Shegan Mahuang Tang significantly reduces TCM syndrome scores and shortens the duration of cough,expectoration,fever and lung rales in children with MPP of wind-cold blocking lung type,making it worthy of clinical promotion.
